维生素D缺乏与冠心病和炎症标志物相关性研究 被引量:8

Study on correlation between vitamin D deficiency with coronary heart disease and inflammatory markers

摘要 目的探讨维生素D缺乏和冠心病发病及炎症因子表达间的相关性。方法选取2014年6月~2016年6月就诊于福建医科大学附属南平第一医院心内科行冠状动脉造影检查400例患者作为研究对象,根据造影结果分为冠心病组294例和对照组106例,冠心病组包括单支病变组104例、双支病变组98例、三支病变组94例。测定和比较四组患者血清25-羟维生素D(25-OHD)、白介素-6(IL-6)、白介素-10(IL-10)和超敏C反应蛋白(hs-CRP)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、三酰甘油(TG)和空腹血糖(FPG)水平;分析25-OHD和各生化检测指标间相关性。结果单支病变组、双支病变组和三支病变组患者25-OHD表达均≤20 ng/dl,低于对照组患者,差异有统计学意义(P<0.01)。单支病变组、双支病变组和三支病变组患者IL-6、IL-10和hs-CRP表达高于对照组患者,差异有统计学意义(P<0.05)。单支病变组、双支病变组和三支病变组患者TC、LDL-C、TG和FPG表达显著高于对照组患者(P<0.01),HDL-C表达显著低于对照组患者(P<0.05)。单支病变组25-OHD表达和IL-6、IL-10、TC、TG、hs-CRP表达呈负相关性(P<0.05)。双支病变组和三支病变组25-OHD表达和IL-6、IL-10、TC、TG、hs-CRP、FPG及LDL-C表达呈负相关(P<0.05),和HDL-C呈正相关(P<0.05)。IL-6、IL-10、TC、TG、hs-CRP、HDL-C及LDL-C均是维生素D缺乏的影响因素(P<0.05)。结论维生素D缺乏和冠心病发病及患者血液炎症因子具有相关性。 Objective To investigate the correlation between vitamin D deficiency and coronary heart diseaseand expression of inflammatory factors.Methods400patients received coronary angiography were selected as the research object from June2014to June2016.They were divided into coronary group(n=294)and control group(n=106)according to angiography results.The coronary group included single vessel group with104patients,double vessel group with98patients and three vessel group with94patients.The expression level of serum25-hydroxyl vitamin D(25-OHD)were measured and compared.The expression level of interleukin-6(IL-6),interleukin-10(IL-10)and high sensitive C reactive protein(hs-CRP)were measured and compared.The expression levels of Total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDLC),three acyl glycerol(TG)and fasting plasma glucose(FPG)were measured and compared.The correlation between25-OHD and biochemical indexes were analyzed.Results The expression of25-OHD of single vessel group,double vessel group and three vessel group was less than or equal to20ng/dl and was significantly lower than that of control group(P<0.01).The expressions of IL-6,IL-10and hs-CRP of single vessel group,double vessel group and three vessel group were significantly higher than those of control group’s(P<0.05).The expressions of TC,LDL-C,TG and FPG of three groups above were significantly higher than those of control group(P<0.01).The expressions of HDL-C of three groups above were significantly lower than that of control group(P<0.05).The expression of25-OHD had a negative correlation with expression level of IL-6,IL-10,TC,TG and hs-CRP in the single vessel group(P<0.05).There were negative correlations between25-OHD with IL-6,IL-10,TC,TG,hs-CRP,FPG,and LDL-C(P<0.05),and there was a positive correlation between25-OHD with HDL-C in double vessel group and three vessel group(P<0.05).IL-6,IL-10,TC,TG,hs-CRP,HDL-C and LDL-C were the influencing factors of vitamin D deficiency(P<0.05).Conclusion Vitamin D deficiency and coronary heart disease and blood inflammation factors are related.
