摘要
宁波市契约式家庭医生制签约服务于2015年全面实施,要求社区卫生机构围绕"保基本、强基层、建机制"的中心任务,以居民需求为导向,提供连续、综合、个性化的健康管理。如何持续、有效地开展此项工作,为分级诊疗的顺利推行打稳基础,是摆在全体基层医疗机构的难题。古林镇卫生院坚持以服务为导向,在理念、推行、绩效分配上深入探索家庭医生新模式,取得初步成效。
