摘要
目的探讨高效价冷凝集素对ABO血型正反定型和电子交叉配血试验的影响。方法选择2015年11月至2016年10月该院因紧急情况输血的500例患者作为研究对象,观察电子交叉与血清交叉主次侧配型结果,不同温度下ABO血型正反定型与电子交叉配血试验的结果,冷凝集素试验红细胞凝集情况及交叉配血结果。结果电子交叉配血与血清学交叉配血相比,主侧结果差异无统计学意义(P>0.05),次侧结果差异有统计学意义(P<0.05)。电子交叉配血时,20℃时有18例出现凝集,明显多于37℃时(P<0.05)。患者血型在正定型A型,反定型B型,O型均有凝集出现,且在20℃时O型与自身对照也有凝集出现。冷凝集素试验结果,当温度低于31℃时会出现红细胞凝集,当低于4℃时,可见明显凝集,但当温度回升到37℃时,凝集消失。结论高效价冷凝集素在低温条件下会促进红细胞凝集,干扰ABO血型正反定型和电子交叉配血试验的结果,从而影响临床输血治疗。
ObjectiveTo study effect of high titer cold lectin on positive and negative typing of ABO blood group and electron cross matching test.MethodsA total of500patients needed emergency blood transfusion were selected from November2015to October2016in People′s Hospital of Qinbei District.To observe the results of electron crossover and serum cross primary and secondary matching,ABO blood group positive and negative and electronic cross matching experimental results at different temperatures,cold agglutinin test erythrocyte agglutination and cross matching results.ResultsThere was no significant difference on the main side between the electronic cross matching and serological cross matching(P>0.05),but the difference was significant on secondary side(P<0.05).In electron cross test,18cases appeared agglutination at20℃,which was obviously more than that at37℃(P<0.05).Patients blood in the positive stereotypes A type,anti stereotypes B type,O type all appeared agglutination,and at20℃O type and self control also appeared agglutination.Cold agglutinin test results showed that when the temperature was lower than31℃,red cell agglutination occurred,when the temperature was lower than4℃,visible agglutination,but when the temperature rose to37℃,agglutination disappeared.ConclusionHigh titer cold lectin could promote the aggregation of red blood cell at low temperature conditions,and effect the results of ABO blood group positive and negative and electronic cross matching test,then effect transfusion therapy.
