摘要
目的:探讨早期综合护理干预对早产儿呼吸暂停的临床应用价值。方法:选取新生儿重症监护室(NICU)收治的100例早产低体重儿作为研究对象,在排除先天性畸形、严重感染等前提下,随机分为观察组和对照组,两组患儿男女性别、初生胎龄、出生阿氏评分、出生体质量、娩出方式等一般资料组间比较,差异无统计学意义(P>0.05)。对照组采用常规护理方案进行护理干预,观察组在采用常规护理干预方案基础上采用综合护理干预措施进行护理干预,观察两组发生原发性呼吸暂停的情况。结果:对照组发生原发性呼吸暂停22例,发生率为44.12%;观察组发生原发性呼吸暂停4例,发生率为8.82%,两组发生原发性呼吸运动暂停比例经统计学分析比较,差异有明显统计学意义(P<0.05)。结论:由于初生早期早产低体重儿呼吸中枢的发育极度不成熟,以外周低级神经反射弧处主导地位,非常容易受外界环境因素刺激影响,出现进食前后、身体体位与外周环境温度等相关因素导致的原发性呼吸运动暂停,临床上通过早期采取综合护理干预措施,可以有效地减少外界因素导致的早产儿原发性呼吸暂停的发生,值得在临床中推广应用。
Objective To explore the clinical value of early comprehensive nursing intervention on premature infants with apnea.Method Neonatal intensive care unit(NICU)from100cases of premature and low birth weight infants as the research object,on the premise of excluding congenital malformation,severe infection,were randomly divided into observation group and control group,two groups of children with gender,gestational age,newborn birth Apgar score,birth body quality,delivery mode of general data between groups,the difference was not statistically significant(P>0.05).The control group was given routine nursing program,and the observation group was given comprehensive nursing intervention based on routine nursing intervention.Nursing intervention was carried out to observe the occurrence of primary apnea in two groups.Results The control group had primary apnea in22cases,the incidence rate was44.12%;the observation group had primary apnea in4cases,the incidence rate was8.82%,two groups had primary respiratory motion to suspend the proportion of the statistical analysis and comparison,there was significant difference(P<0.05).Conclusion The newborn early preterm low birth weight infants respiratory center development is extremely immature,low peripheral nerve reflex arc at the leading position,very easy to stimulate the impact of environmental factors,resulting in related factors before and after eating,body posture and peripheral temperature of primary respiratory movement through early clinical pause,take comprehensive the nursing intervention measures can effectively reduce the external factors lead to the primary apnea of premature infants,it is worth in clinical application.
