摘要
目的探索2型糖尿病(type 2 diabetes mellitus,T2DM)患者生活方式管理体验,为实施护理干预及提高血糖管理效果提供依据。方法采用质性研究中扎根理论研究方法,通过半结构式深入访谈法收集资料,经过开放式登录、轴心式登录、选择式登录三级登录法并结合持续性比较法分析资料,形成T2DM患者生活方式管理体验及应对策略概念模型。结果在该概念模型中,核心类属为足够的认知,同时生命质量、自我管控、知识更新及阶段适应为与核心类属相辅相成的下位类属,而认知阶段和对应的情感变化伴随整个疾病的发展过程。结论护理人员要以患者是否有足够的认知为出发点和落脚点,根据概念模型对T2DM患者进行由内而外的深化教育并实施护理措施,以提高该类患者的生活方式管理质量,优化其临床及院外整体护理模式。
Objective The objective of this study was to explore the lifestyle management experience of type2diabetes mellitus(T2DM)patients,and to provide basis for implementing nursing intervention and improving the management of blood glucose.Methods This study adopted grounded theory,collected data by semi-structure and in-depth interviews,analyzed them by tertiary login methods(including open coding,axial coding and selective coding)combined with constant comparative method,and formed a concept model.Results In this model,the core category was comprised of full cognition,and the subordinate category supplementing to the core category consisted of quality of life,self-management,knowledge renewal and periodical adaptation.The cognition stage and the relevant emotional fluctuation were accompanied during the entire course of disease development.Conclusions The nursing measures and comprehensive education are supposed to be implemented based on the patients'cognition and the concept model,with a view to improving the quality of T2DM patients'management and optimizing the clinical and pre-hospital care mode.
