摘要
目的对比观察全髋关节置换术与半髋关节置换术治疗股骨颈骨折对老年患者髋关节功能恢复的影响,旨在为临床治疗方案的选择提供参考。方法回顾性分析64例于本院确诊为股骨颈骨折老年患者的临床资料,按照手术方式分为观察组(34例)和对照组(30例)。观察组患者行全髋关节置换术治疗,对照组患者行半髋关节置换术治疗,术后12个月进行随访,通过比较两组患者髋关节Harris评分判断两种治疗方法对患者髋关节功能恢复的影响,并对比两组患者手术一般情况(手术时间,术中出血量、引流量及术后下地时间)及术后并发症发生情况,以判断两种治疗方法的应用价值。结果对照组手术时间,术中出血量、引流量及术后下地时间均少于观察组,差异具有统计学意义(P<0.05);观察组Harris评分优良率明显高于对照组(88.24%vs.76.66%),差异具有统计学意义(P<0.05);观察组并发症发生率明显低于对照组(8.82%vs.26.67%),差异具有统计学意义(P<0.05)。结论与半髋关节置换术相比,全髋关节置换术有助于术后老年股骨颈骨折患者术后髋关节功能恢复,临床医师应根据患者的实际情况,合理制定手术方案,对于体质较弱的患者首选半髋关节置换术,对于手术耐受力高的患者可选择全髋关节置换术治疗。
Objective To observe the effect of stotal hip and hemiarthroplasty on the recovery of hip joint function in elderly patients with femoral neck fracture.Methods Retrospectively analyzed the clinical data of patients with femoral neck fracture in our hospital,according to the surgical approach,the patients were divided into observation group(34cases,total hip arthroplasty)and control(30cases,hemiarthroplasty).Followed up for12months,the Harris score,general surgery status and postoperative complications were compared between the two groups.Results The control group had less operation time,intraoperative blood loss,drainage volume and postoperative time than the observation group,the Harris score in the observation group was significantly higher than that in the control group(88.24%vs.76.66%),the incidence of complications in the observation group was significantly lower than that in the control group(8.82%vs.26.67%),the difference were statistically significant(P<0.05).Conclusion Compared with hemiarthroplasty,total hip arthroplasty contributes to postoperative recovery of hip function in elderly patients with femoral neck fracture.
