摘要
目的探讨个性化护理干预对妊娠期糖尿病患者血糖水平以及妊娠结局的影响。方法将本院收治的妊娠期糖尿病患者180例随机分为对照组与观察组各90例,对照组在治疗过程中接受常规护理干预,观察组接受个性化护理干预,对研究对象行随访直至分娩,分析2组患者干预前后的血糖水平以及妊娠结局。结果护理干预前,2组患者空腹血糖(FPG)、餐后2 h血糖(2 h-PG)、糖化血红蛋白(Hb A1c)比较差异无统计学意义(P>0.05);护理干预后,观察组FPG、2 h-PG、Hb A1c均低于对照组(P<0.05);观察组患者羊水过多、妊娠高血压疾病、泌尿生殖系统感染发生率低于对照组(P<0.05);2组患者剖宫产、产后出血发生率相比差异无统计学意义(P>0.05)。观察组围产儿低血糖、呼吸窘迫综合征、巨大儿发生率低于对照组(P<0.05)。2组围产儿畸形发生率比较差异无统计学意义(P>0.05)。结论妊娠期糖尿病患者应用个性化护理干预效果确切,血糖控制效果好,可以获得较好妊娠结局。
Objective To explore the influence of individualized nursing intervention on blood glucose level and pregnancy outcomes in gestational diabetes mellitus patients.Methods A total of 180 gestational diabetes in our hospital were randomly divided into control group(90 cases)and observation group(90 cases),given routine nursing intervention and individualized nursing intervention,respectively.And all the patients were followed up until delivery.Blood glucose levels and pregnancy outcomes before and after intervention were compared.Results Before intervention,fasting blood glucose(FPG),2 h postprandial blood glucose(2h-PG),glycosylated hemoglobin(HbA1c)of two groups had no significant difference.After nursing intervention,the observation group had lower FPG,2h-PG,HbA1c levels than the control group(P<0.05).The observation group had lower incidences of polyhydramnios,pregnancy induced hypertension,urinary system infection than the control group(P<0.05).The incidence of cesarean section and postpartum hemorrhage showed no statistically significant difference between the two groups(P>0.05).The observation group had lower perinatal hypoglycemia,respiratory distress syndrome and macrosomia than that in the control group(P<0.05).The incidence of perinatal malformation had no statistically significant difference between the two groups P>0.05).Conclusion Individualized nursing intervention in gestational diabetes mellitus is effective,and has good effect in blood glucose control and better pregnancy outcomes.
