摘要
目的比较游乐园式功能锻炼与视频功能锻炼方法对Gartland Ⅲ型儿童肱骨髁上骨折术后肘关节功能恢复效果。方法收集本科室自2014年1月至2015年12月肱骨髁上骨折患儿83例,男54例、女29例,年龄为2~14岁,平均6.5岁。伤后至就诊时间为(7.1±2.8)h。根据数字随机法将患儿分为:视频组42例,术后康复运用视频功能锻炼方法;游乐园组41例,术后康复运用游乐园式功能锻炼方法。两组患儿基本资料(性别、年龄、骨折类型及治疗方法)比较,差异均无统计学意义(均P>0.05)。比较两组患儿对两种功能锻炼方法配合的依从性(根据2000年美国Mariond护理评价分类系统进行设计)和肘关节功能恢复程度(采用Mayo肘关节功能评分及肘关节ROM值)。结果游乐园组患儿依从性较高,术后肘关节功能恢复快,并且平均锻炼时间及并发症发生率均低于视频组,差异均有统计学意义(均P<0.05)。结论游乐园式功能锻炼指导Gartland Ⅲ型儿童肱骨髁上骨折术后进行功能锻炼既可以促进患儿肘关节功能恢复,又能够减少术后并发症的发生,值得在小儿骨科广泛开展。
Objective To discuss the clinical effectiveness of function training in a playing recovery way for elbow joint recovery after GartlandⅢsupracondylar fracture of humerus in children.Methods From January 2014 to December 2015,83 patients with supracondylar fracture of humerus,including 54 males and 29 females,were hospitalized in our medical center,with age ranging from 2 to 14 years(mean 6.5 years).Time interval between fracture and treatment was(7.1±2.8)h.According to random number table,the cases were divided into two groups,42 patients in watching video recovery group and 41 in playing recovery group.Basic information(sex,age,type of fracture and treatment method)of the two groups had no statistically significant difference(P>0.05).The present research aimed to compare compliance based on Mariond America nursing outcomes classification and recovery of elbow function based on Mayo elbow function grade and ROM.Results Compliance in the playing recovery group was better,function recovery of the elbow was quicker,average time for physical exercise was shorter,and complications were less than in the watching video recovery group,with the difference being statistically significant between two groups.Conclusion The present research showed that function training in a playing recovery way for elbow joint recovery after GartlandⅢsupracondylar fracture of humerus could not only promote elbow function recovery,but also reduce post-operative complications.Therefore,its wide application in pediatric orthopedics is very meaningful.
