摘要
目前,大力发展全科医学、建立一支高水平的全科医生队伍是增强基层卫生服务能力的关键。临床决策支持系统(CDSS)是一种改善临床决策制定的信息系统,通过卫生信息化技术将患者临床资料和医学知识相结合,辅助临床医务人员制定临床决策。在基层卫生信息系统中引入CDSS,可弥补基层医疗卫生机构先进设施相对缺乏、基层全科医生诊疗经验不足,起到提升全科医生服务水平的作用。本研究结合国内外CDSS发展现状,探讨了应用CDSS在促进全科医生发展、提升全科医生诊疗水平中的作用和实际意义,指出:CDSS可提升全科医生的疾病诊疗能力,帮助全科医生参与多种共存病的管理,提示人群高危因素及指导疾病筛查,优化转诊模式,帮助全科医生科学合理地应用指南,促进全科医生自我学习等。
Vigorously developing general practice and establishing a high-level team of general practitioners(GPs)are the keys to strengthening health service capabilities in grassroots level at present.Combining clinical data and medical knowledge with health information technology,clinical decision support system(CDSS)is an information system to assist clinical staffs in making clinical decisions.The introduction of CDSS in grassroots health information system can relatively cover the shortage of the lack of advanced facilities and experienced GPs in primary health care institutions,which plays an important role in improving health service capabilities of GPs.Based on the current status of CDSS both at home and abroad,this study explored the role and practical significance of CDSS in improving the development of GPs and enhancing their clinical practice.It pointed out that CDSS could enhance GPs'abilities to diagnose and treat diseases,and help GPs to participate in the management of multiple comorbidities as well as to apply scientific and rational clinical guidelines with the idea of self-learning.CDSS could also prompt risk factors,guide the screening for diseases,and optimize the referral mode.
