摘要
目的探讨乌司他丁联合血必净注射液对急性百草枯(PQ)中毒患者动脉血气指标及多器官功能障碍综合征(MODS)发生率的影响。方法选取2014年5月—2016年6月南阳市中心医院92例急性PQ中毒患者,随机数字表法分组,各46例。对照组予以常规治疗,观察组于常规治疗基础上予以乌司他丁+血必净注射液治疗。统计对比两组治疗前后动脉血气指标[动脉血氧分压(PaO2)、二氧化碳分压(PaCO2)、碳酸氢根(HCO3-)],对比两组预后情况(肺纤维化、MODS发生率及死亡率)。结果治疗后观察组PaO2、HCO3-水平高于对照组,且PaCO2水平低于对照组,差异有统计学意义(P<0.05);观察组肺纤维化发生率50.00%(23/46)、MODS发生率67.39%(31/46)及死亡率41.30%(19/46)均低于对照组73.91%(34/46)、86.96%(40/46)、63.04%(29/46),差异有统计学意义(P<0.05)。结论乌司他丁与血必净注射液联合治疗急性PQ中毒,可降低患者肺纤维化、MODS发生率及死亡率,能改善其动脉血气。
Objective To investigate the effects of ulinastatin combined with Xuebijing Injection on the arterial blood gas index and the incidence of multiple organ dysfunction syndrome(MODS)in patients with acute paraquat(PQ)poisoning.Methods A total of 92 cases of acute PQ poisoning treated during May 2014 and June 2016 were randomly divided into two groups.The control group(n=46)received conventional treatment,while the observation group(n=46)was treated with ulinastatin and Xuebijing Injection on the basis of routine treatment.The arterial oxygen pressure(PaO 2),partial pressure of carbon dioxide(PaCO 2),bicarbonate(HCO 3-)before and after treatment,pulmonary fibrosis,MODS incidence and mortality were compared between the two groups.Results After treatment,the levels of PaO 2 and HCO 3-were higher,and the level of PaCO 2 was lower in the observation group than in the control group,and the differences were statistically significant(P<0.05).The incidence of pulmonary fibrosis,MODS,and mortality were 50%(23/46),67.39%(31/46),and 41.30%(19/46)in the observation group,and 73.91%(34/46),86.96%(40/46)and 63.04%(29/46)in the control group(P<0.05).Conclusion In the treatment of acute PQ poisoning,ulinastatin combined with Xuebijing Injection can reduce the incidence of pulmonary fibrosis,MODS and mortality,and improve the arterial blood gas.
