摘要
目的探讨多舱室条件下爆炸冲击致骨折伤伤情特点,为密闭舱室爆炸时内部人员的防护及救治提供理论依据。方法选取76只新西兰大白兔,赛拉嗪镇静后随机编号按距爆源不同距离放置。在钢板构建的多舱室模型中,使用大小2种常规口径舰炮实、裸弹作为爆源进行爆炸实验。死亡动物当场解剖,存活动物饲养观察24 h后处死解剖,均行X线检查。结果实验动物死亡率59.2%;骨折发生率55.3%,以四肢为主。小口径实、裸弹与大口径实、裸弹致骨折率及邻舱机柜前、后整体骨折率比较,差异均有统计学意义(P<0.05)。舱室爆炸伤实验动物直接死亡率高,晚期死亡率低。结论舰船多舱室结构中,舱室整体结构及内部机柜结构对兔有一定防护作用;舰船舱室内爆炸伤多为多发伤,这与舱室构造、结构的强度以及冲击波在舱室内的反射、折射有关;爆炸致兔骨折伤以四肢为主,二次破片及抛掷碰撞是骨折的主要机制。现场仔细检伤,早期骨折固定,尽快实施治疗。
Objective To explore the characteristics of the rabbit fracture in multiple cabins explosion,in order to provide theoretical basis for the protection and management of the wounded.Methods A total number of 76 New Zealand white rabbits were sedated by xylazine,the random numbers place at different distance from the burst source.Two conventional caliber naval gun real/naked shells were used as the explosive source in multiple cabins,which was build by steel plates.Dead animals were dissected while the survived animals were dissected after observing for 24 hours,all of the animals were taken the X-ray examination.Results Experimental animal mortality was 59.2%,and the incidence of fractures was 55.3%in multiple cabins after explosion,limbs were given priority to the fracture.The fracture rates between different caliber ammunition groups were significant difference(P<0.05),the fracture rates between the front and behind the cabinet in adjacent cabin had significant difference(P<0.05).The immediately mortality rate after explosion was higher,and late mortality rate was lower in blast experiment under the condition of multiple cabins.Conclusion The overall structure of the cabin and the internal cabinet structure have some protective effects on rabbits.Compound injury is the mainly blast injury,which is related to the strength of the structure and shock wave after the reflection and refraction.Limb fracture is the most of the wounded,fragment and throwing collision may be the main mechanism.Careful examination at the explosive site,early fracture fixation,and as soon as possible treatment are the principle for management of rabbit fracture.
