塞来昔布对重度膝关节骨关节炎全膝关节置换术患者滑膜组织环氧合酶-2表达的影响 被引量:3

Effect of Celecoxib on Cyclooxygenase-2 Expression in Synovial Tissue of Patients with Severe Knee Osteoarthritis After Total Knee Arthroplasty

摘要 目的探讨塞来昔布对重度膝关节骨关节炎全膝关节置换术患者的滑膜组织环氧合酶-2表达的影响。方法选取重庆市新桥医院2015年2月至2017年2月收治的接受全膝关节置换术的重度膝关节骨关节炎患者236例,按随机数字表法分为观察组和对照组,各118例。观察组患者术前连服3 d塞来昔布,对照组患者不服用。采用real-timeP CR技术测定滑膜组织环氧合酶-2(COX-2)mRNA及前列腺素E合成酶1(mP GEs)mRNA表达,并测定滑膜组织COX-2及mP GEs mRNA蛋白的浓度及关节液前列腺素E_2(PGE_2),肿瘤坏死因子-α(TNF-α),白细胞介素1β(IL-1β)及血清PGE_2浓度,并进行对比。结果 COX-2 mRNA的表达浓度,观察组为(5.94±3.43)×10^(-4) mol/L,高于对照组患者的(3.47±6.14)×10^(-4) mol/L(P<0.05);两组患者mP GEs mRNA的表达、COX-2 mRNA蛋白和mP GEs mRNA蛋白的表达比较,差异无统计学意义(P>0.05);观察组关节液PGE_2为(237.74±51.97)pg/mL,TNF-α为(34.47±13.74)pg/mL,IL-1β为(60.14±51.16)pg/mL,血清PGE_2为(284.11±79.65)pg/mL,对照组患者分别为(171.21±79.48)pg/mL,(38.87±21.71)pg/mL,(29.98±12.84)pg/mL,(274.74±59.18)pg/mL,两组的差异无统计学意义(P>0.05)。结论全膝关节置换术前服用塞来昔布可抑制COX-2 mRNA的表达,而对mP GEs mRNA和mP GEs mRNA蛋白的表达、COX-2 mRNA蛋白和关节液PGE_2,TNF-α,IL-1β及血清PGE_2浓度均无明显影响。 Objective To investigate the effect of celecoxib on the expression of cyclooxygenase-2(COX-2)in synovial tissue of patients with severe knee osteoarthritis after total knee arthroplasty.M ethods Totally 236 patients with severe knee osteoarthritis u n d er/o in/total knee arthroplasty from February 2015 to February 2017 in Xinqiao Hospital of Chongqin/were selected and divided into the observation/roup and the control/roup accord into the random number table method,118 cases in each/ro u p.The patients in the observation/roup were treated with celecoxib for 3 d before operation,and the patients in the control/o u p were not taken.The expression of COX-2 mRNA and prostaglandin E synthase 1(m PG Es)mRNA in synovial tissue were determined by real-time P C R,and the concentration of COX-2 mRNA pretien and mPGEs mRNA pretien in synovial tissue and the concentration synovial fluid and serum PGE2 were and compared.R esults The expression concentration of COX-2 mRNA in the observation/roup was(5.94±3.43)x 1 0_4 mol/L,which was si/nificantly higher than(3.47±6.14)x 1 0-4 m o l/L in the control/roup(P<0.0 5).There was no significant difference in the expression of mPGEs mRNA,COX-2 mRNA protein and mPGEs mRNA protein between the two groups(p>0.0 5).The concentration of PGE2[(2 3 7.7 4±5 1.9 7)p g/m L],T N F-a[(3 4.47±1 3.7 4)p g/m L],I L-i p[(6 0.1 4±5 1.1 6)pg/m L]and serum PGE2[(284.11±79.65)pg/mL]in the joint fluid of the observation group had no significant difference from(171.21土79.48)pg/mL,(38.87±2 1.71)pg/mL,(29.98±12.84)pg/mL,(274.74±5 9.18)pg/mL of the control group(P>0.0 5).Conclusion Taking celecoxib before total knee arthroplasty can inhibit the expression of COX-2 mRNA,but it has no significant effect on the expression of mPGEs mRNA,mPGEs mRNA protein,COX-2 mRNA protein and the concentration of PGE2,TNF-a,II,-1 and serum PGE2.
