摘要
目的分析高血压脑出血患者发病早期血脑利钠肽(BNP)和同型半胱氨酸(Hcy)水平的变化及与转归的关系,为临床诊疗提供参考。方法回顾性分析笔者诊治的160例高血压脑出血患者临床资料,检测患者脑出血后24 h内血BNP和Hcy水平,比较不同严重程度患者血清BNP和Hcy水平变化。结果随着脑出血严重程度的增加,患者BNP和Hcy阳性率及水平逐步升高,且轻、中和重度患者之间存在显著差异(P<0.05);好转组的BNP和Hcy阳性率明显低于未好转组(P<0.05),且BNP和Hcy均阳性患者未好转的比例明显高于单独BNP阳性(42.8%)或单独Hcy阳性(37.7%)患者(P<0.05)。结论高血压脑出血患者BNP和Hcy水平越高病情越重,二者均阳性患者的预后更差。
Objective To analyze the changes in brain natriuretic peptide(BNP)and homocysteine(Hcy)levels of hypertensive cerebral hemorrhage patients and the relationship with prognosis in order to provide reference for clinical diagnosis and treatment.Methods A retrospective analysis was perfonned on clinical results of a total of 160 hypertensive cerebral hemorrhage patients diagnosed and treated by the author.The BNP and Hcy levels of the patients within 24 hours after the cerebral hemorrhage were examined and the changes in serum BNP and Hcy levels of patients with all degrees of disease were compared.Results Both positive rates and levels of BNP and Hcy of the patients gradually increased along with the increase in the severity of cerebral hemorrhage,and there were significant differences(P<0.05)among the patients with mild,moderate and severe diseases;positive rates of BNP and Hcy of the improved group were greatly lower than those of the unimproved group(P<0.05)while the proportion of patients with positive BNP and Hcy was significantly higher than those with positive BNP(42.8%)or Hcy(37.7%)(P<0.05).Conclusion Higher BNP and Hcy levels of hypertensive cerebral hemorrhage patients indicate more severe disease.The prognosis is worse for patients with positive BNP and Hcy.
