药物过量使用性头痛患者脑干结构变化初步磁共振成像评估

Altered Brainstem Volume in Medication-overuse Headache:A Pilot Magnetic Resonance Imaging Evaluation

摘要 目的探讨药物过量使用性头痛(MOH)患者脑干结构变化。方法对36例MOH患者及32名正常对照(NC)进行脑部高分辨结构成像,将脑干结构分割为中脑、脑桥及延髓,并计算各分部体积。结果 MOH患者中脑体积(5.80±0.53)ml小于NC(6.14±0.67)ml(t=2.36,P=0.02)。MOH患者脑桥、延髓及整个脑干体积[(13.13±1.42)ml、(4.55±0.51)ml、(23.48±2.23)ml]与NC[(13.67±1.61)ml、(4.66±0.44)ml、(24.47±2.56)ml]比较差异无统计学意义(t_(脑桥)=1.47,P_(脑桥)=0.15;t_(延髓)=0.93,P_(延髓)=0.35;t_(脑干)=1.71,P_(脑干)=0.09)。结论中脑体积减小可能是MOH痛觉传导通路的特异性改变,脑干体积自动定量计算方法可以作为MOH患者脑干容积变化的评价工具。 Objective To investigate the altered brainstem volume in patients with medication-overuse headache(MOH).Methods The high-resolution structural images were obtained from 36 MOH patients and 32 normal controls(NC).The brainstem was segmented into midbrain,pons,and medulla,whose volume were measured respectively.Results There was a significantly smaller midbrain volume in MOH patients[(5.80±0.53)ml]than that in NC[(6.14±0.67)ml](t=2.36,P=0.02).The volumes of pons,medulla,and whole brainstem showed no significant difference in MOH patients[(13.13±1.42)ml,(4.55±0.51)ml,and(23.48±2.23)ml,respectively]compared with those in NC[(13.67±1.61)ml,(4.66±0.44)ml,and(24.47±2.56)ml,respectively](t pons=1.47,P pons=0.15;t medulla=0.93,P medulla=0.35;and t brainstem=1.71,P brainstem=0.09,respectively).Conclusion A smaller midbrain volume may be one of the specific features of pain pathway in MOH,and the automated brainstem subfield segmentation and volumetry may be useful tools for evaluating brainstem alternation in MOH patients.
