摘要
目的观察头针百会透太阳联合阳明经穴位埋线治疗中风后痉挛性偏瘫的临床观察。方法将90例中风后痉挛性偏瘫患者随机分为3组。头针组30例予头针百会透太阳针法治疗;头针联合体针组(以下简称体针组)30例予头针联合体针治疗,头针疗法同头针组,体穴取穴以患侧阳明经穴为主,上肢取肩髃、曲池、外关及合谷,下肢取梁丘、足三里、阳陵泉、解溪、昆仑、内庭及太冲,均采用常规针刺治疗;头针联合埋线组(以下简称埋线组)30例予头针联合穴位埋线治疗,头针疗法同头针组,穴位埋线治疗取穴同体针组。3组均治疗2个月后统计疗效。比较3组治疗前及治疗1、2个月后肢体功能恢复情况及偏瘫分期变化情况,采用简化Fugl-Meyer运动功能评分法(FMA)对患者肢体功能改善情况进行评定,并采用Brunnstrom偏瘫运动功能障碍分期标准对患者偏瘫状况进行分期评价。结果埋线组总有效率93.3%,体针组总有效率86.7%,头针组总有效率73.3%,埋线组疗效优于体针组和头针组(P<0.05);3组治疗1、2个月后FMA评分与本组治疗前比较均明显升高(P<0.05),且埋线组治疗1、2个月后FMA评分均高于体针组和头针组治疗后同期水平(P<0.05);埋线组治疗1、2个月后Brunnstrom偏瘫分期改善情况均优于体针组和头针组(P<0.05)。结论头针百会透太阳联合阳明经穴位埋线治疗中风后痉挛性偏瘫临床效果显著,可明显促进患者肢体功能恢复,改善患者偏瘫状态,提高临床疗效。
Objective To observe clinical effects of scalp acupuncture penetrating acupuncture from Baihui and Taiyang combined with Yangming meridian acupoint catgut embedding therapy on spastic hemiplegia after stroke.Methods 90 patients with spastic hemiplegia after stroke were randomly divided into three groups.30 cases in scalp acupuncture group were treated by scalp acupuncture penetrating acupuncture from Baihui and Taiyang,30 cases in scalp acupuncture combined with body acupuncture group(hereinafter referred to as body acupuncture group)were treated by scalp acupuncture combined with body acupuncture,the scalp acupuncture therapy is the same as the scalp acupuncture group.The acupoints of body acupuncture is taken from the main point of Yangming meridian in the affected side.The Jianyu,Quchi,Waiguan,Hegu acupoints were selected on the upper limb,Liangqiu,Zusanli,Yangliquan,Jiexi,Kunlun,Neiting,Taichong acupoints were selected on the lower limb,both treated by routine acupuncture treatment.30 cases of scalp acupuncture combined with catgut embedding group(hereinafter referred to as catgut embedding group)were treated by scalp acupuncture combined with acupoint catgut embedding therapy,the scalp acupuncture therapy were same as the scalp acupuncture group,the acupoints selection were same as body acupuncture group,but they were treated by acupoint catgut embedding therapy.After 2 months of treatment,the statistical results were compared.The changes of limb function recovery and hemiplegic stage changes were compared before and after treatment for 1 and 2 months.Simplified Fugl-Meyer motor function score(FMA)was used to evaluate the limb function improvement of the patients.Brunnstrom stage criteria for hemiplegia and motor dysfunction were used to assess the stage of patients with hemiplegia.Results The total effective rate was 93.3%in the catgut embedding group,86.7%in the body acupuncture group,and 73.3%in the scalp acupuncture group.The catgut embedding group was superior to the body acupuncture group and the scalp acupuncture group(P<0.05);after 1 and 2 months,the FMA score of the 3 groups was significantly increased(P<0.05),and the FMA score of the catgut embedding group after 1 and 2 months was higher than that of the body acupuncture group and the scalp acupuncture group(P<0.05).The improvement of the Brunnstrom hemiplegia stage in the catgut embedding group after 1 and 2 months was better than the body acupuncture group and the scalp acupuncture group(P<0.05).Conclusion Scalp acupuncture penetrating acupuncture from Baihui and Taiyang combined with Yangming meridian acupoint catgut embedding therapy on spastic hemiplegia after stroke has a significant clinical effect,can obviously promote the recovery of limb function,improve the hemiplegia and improve the clinical effect.
