摘要
目的对比分析经胸超声心动图(trans thoracic echocardiography,TTE)与CT血管成像对肺静脉异位引流(APVC)的诊断价值,总结误诊及漏诊原因。方法回顾性分析我院35例经心外科手术证实的APVC患儿TTE及CTA图像特点,将TTE及CTA诊断结果与手术结果进行对比分析。结果 35例患儿中,手术证实完全型肺静脉异位引流28例,其中心上型18例,心内型6例,心下型2例,混合型2例。部分型肺静脉异位引流7例;超声诊断完全型26例,漏诊2例。诊断部分型7例,误诊1例,超声诊断符合率91.4%;CTA诊断完全型27例,部分型7例,ASD1例,经手术证实漏诊1例,误诊1例,诊断准确率为94.3%。结论 TTE是APVC首选检查方法,能对APVC的诊断和分型做出明确诊断,CTA对APVC及心外大血管尤其对APVC肺静脉开口显示不清,易误诊为单纯ASD的患儿有独到的优势。
Objective To comparatively analyse of transthoracic echocardiography(trans thoracic,echocardiography,TTE)and CT angiography on pulmonary venous drainage the diagnostic value,and to summarize the causes of misdiagnosis and missed diagnosis.Methods TTE and CTA images of 35 patients with APVC confirmed by cardiac surgery in our hospital were retrospectively analyzed,and the results were compared with the results of TTE and CTA diagnosis.Results Among 35 cases,28 cases were total anomalous pulmonary venous drainage,18 cases were central type,6 cases were cardiac type,2 cases were cardiac type and 2 cases were mixed type.Partial anomalous pulmonary venous drainage was found in 7 cases;ultrasonic diagnosis showed 26 cases were complete type,2 cases were missed diagnosis.Part of the diagnosis were 7 cases,1 case was misdiagnosis,ultrasound diagnosis coincidence rate was 91.4%;CTA diagnosed 27 cases of complete type,7 cases of partial type,1 case of ASD,1 case confirmed by surgery,1 case of misdiagnosis,the diagnostic accuracy rate was 94.3%.Conclusion TTE APVC is the preferred method of examination,diagnosis and typing of APVC in diagnosis of CTA.APVC and excardiacvascular especially for APVC pulmonary vein opening unclear misdiagnosed as simple ASD patients have unique advantages.
