摘要
目的:分析一次法根管治疗联合全冠修复对合并牙髓炎隐裂磨牙的临床疗效。方法:选取2016年1月-2017年2月本院确诊为隐裂性牙髓炎磨牙的患者170例(170颗患牙)为研究对象。按照治疗方法的不同将其分为对照组和分析组,各85例。对照组在常规根管治疗后进行全冠修复,分析组在一次性根管治疗后进行全冠修复。两组患者治疗后3个月、半年和1年进行复诊,统计其临床疗效及术后疼痛情况。结果:两组患者的患牙失败率均随着时间的推移而逐渐上升;不同复诊时间,两组患者患牙成功率、失败率比较差异均无统计学意义(P>0.05);分析组疼痛0级患者多于对照组,Ⅰ、Ⅲ级患者均少于对照组,比较差异均有统计学意义(P<0.05);两组疼痛Ⅱ级患者比较差异无统计学意义(P>0.05)。结论:合并牙髓炎隐裂磨牙患者采用一次法根管治疗联合全冠修复治疗,可提高短期成功率,其疗效随着时间推移而下降,且可降低治疗后的疼痛程度,值得临床推广。
Objective:To analyze the one-time root canal therapy combined with full crown restoration in treatment of molars with pulpitis cleft.Method:A total of 170 patients(170 teeth)who had been diagnosed as cracked dental pulpitis in our hospital from January 2016 to February 2017 were selected as the subjects.According to different treatment methods,they were divided into control group and analysis group,85 cases in each group.The control group received full crown restoration after routine root canal treatment,and analysis group received full crown restoration after one-time root canal therapy.They were followed up for 3 months,half a year and 1 year after treatment,the clinical efficacy and postoperative pain were analyzed.Result:The failure rate of two groups of patients gradually increased with time,and the success rate and failure rate of two groups were compared,the differences were not statistically significant(P>0.05).The patients pain of grade 0 was more than that of control group,the patients of gradeⅠandⅢwere less than those of control group,the differences were statistically significant(P<0.05).The patients pain of gradeⅡin two groups were compared,the difference was not statistically significant(P>0.05).Conclusion:One-time root canal therapy combined with full crown restoration for treatment of molars with pulpitis cleft,can improve short-term success rate,and its curative effect decreases with time,and it can reduce the degree of pain after treatment,which is worthy of clinical promotion.
