双胎妊娠并发症孕妇血管阻力及子宫动脉血流参数纵向研究 被引量:11

Longitudinal Study of Vascular Resistance and Uterine Artery Blood Flow Parameters in Pregnant Women with Twin Pregnancy Complications

摘要 目的探讨双胎妊娠并发症孕妇血管阻力及子宫动脉血流参数的变化规律。资料与方法选择2013年1月-2015年1月在首都医科大学附属北京妇产医院建档的20例双胎妊娠并发症孕妇作为研究组,并选择同期50例正常双胎孕妇作为对照组,于妊娠各期行超声心动图、子宫动脉检查及测量血压,对其总血管阻力、血压及子宫动脉血流行纵向研究。结果对照组妊娠早中期总血管阻力降低(P<0.01),研究组无明显变化(P>0.05);研究组双侧子宫动脉血流参数值及舒张早期切迹随妊娠进展减少(P<0.01),妊娠早中期舒张早期切迹存在率较对照组高(P<0.05),妊娠中期舒张期血压升高(P<0.05),妊娠晚期收缩期血压升高(P<0.05)。结论双胎妊娠并发症孕妇总血管阻力未出现适应性降低,影响胎盘功能导致并发症发生,妊娠中晚期血压及早中期子宫动脉舒张早期切迹存在率较正常双胎孕妇高,可作为预测并发症的候选指标。 Purpose To explore the changes of vascular resistance and uterine artery blood flow parameters in pregnant women with twin pregnancy complications.Materials and Methods Twenty pregnant women with twin pregnancy complications enrolled in Beijing Obstetrics and Gynecology Hospital,Capital Medical University from January 2013 to January 2015 were included in the study group,and 50 normal twin-pregnant women of the same period were selected as the control group.Ultrasonic cardiogram,uterine artery examination and blood pressure measuring were conducted at various trimesters of pregnancy,and total vascular resistance,blood pressure and uterine artery blood flow were longitudinally studied.Results In the control group,the total vascular resistance decreased in the first and second trimesters of pregnancy(P<0.01),with no obvious changes in the study group(P>0.05);the bilateral uterine artery blood flow parameters and early diastolic notch in the study group decreased with the progress of pregnancy(P<0.01).The prevalence of early diastolic notch in the first and second trimesters of pregnancy was higher than that in the control group(P<0.05),with diastolic blood pressure in the second trimester of pregnancy(P<0.05)and systolic blood pressure in the third trimester of pregnancy increased(P<0.05).Conclusion The total vascular resistance in pregnant women with twin pregnancy complications suggested no adaptive reductions which affect the development of placental function,leading to occurrence of complications.The blood pressure of the second and third trimesters of pregnancy and the early notch prevalence in the early and middle stages of uterine artery diastole is higher than that of normal twin-pregnant women,which can be used as candidate predictors of complications.
