摘要
目的调查闽西地区妊娠妇女碘营养水平,建立闽西地区妊娠妇女特异性血清甲状腺激素的正常参考范围,并分析碘营养对甲状腺激素的影响。方法经过严格筛选得到妊娠妇女722例,检测其尿碘及甲状腺功能、甲状腺球蛋白抗体、甲状腺过氧化物酶抗体。结果 722份妊娠妇女的尿碘中位数为161.6μg/L;取尿碘处于适宜状态的孕早期妊娠妇女127例,孕中期妊娠妇女142例,孕晚期妊娠妇女124例,并建立了本地区不同孕期特异性甲状腺激素正常参考范围。根据本研究建立的参考范围,分析不同碘营养下妊娠期甲状腺功能异常发生率,不同碘营养状态下,单纯低T4血症及临床甲减,亚临床甲减发生率比较差异有统计学意义。结论闽西地区妊娠妇女碘营养处于碘适量状态,建立了妊娠期特异性的血清FT3、FT4和TSH的正常参考范围,不同碘营养下,妊娠期甲状腺功能异常发生率不同。
Objective To study iodine level of pregnant women in Western Fujian,establish the gestational specific reference intervals for and thyroxine in Western Fujian and analyze the effect of iodine nutrition on thyroid hormone.Methods 722 cases of pregnant women were screened strictly,and their urine iodine and thyroid function,thyroglobulin antibody and thyroid peroxidase antibody were detected.Results The median urine iodine of 722 pregnant women was 161.6μg/L,127 pregnant women in the early pregnancy,142 pregnant women in the middle pregnancy and 124 pregnant women in the late trimester of pregnancy,and the normal reference range of different pregnancy specific thyroid hormones in the local area was established.According to the reference range established in this study,the incidence of abnormal thyroid function during pregnancy was analyzed under different iodine nutrition.The incidence of simple hypotoxemia of T4,and clinical hypothyroidism,subclinical hypothyroidism in different iodine nutritional states was statistically significant.Conclusion the iodine nutrition of pregnant women in Western Fujian is in a proper state of iodine,and the normal reference range of specific serum FT3,FT4 and TSH in pregnancy is established.Under different iodine nutrition,the incidence of abnormal thyroid function in pregnancy is different.
