不同位置角膜切口对白内障术后角膜散光和视力的影响分析 被引量:1

Analysis of the Impact of Corneal Incision in Different Locations on Corneal Astigmatism and Visual Acuity after Cataract Surgery

摘要 目的探究不同位置角膜切口对白内障术后角膜散光和视力的影响。方法研究对象选取为2016年8月~2017年12月云南省第三人民医院收治的106例白内障患者(106眼),所有患者均采用超声乳化白内障吸除联合人工晶状体植入术治疗,采用数字表法随机分为观察组和对照组各53例,对照组选择颞上(右眼)、鼻上(左眼)做角膜切口,观察组根据曲率检查结果选择在角膜最大子午线轴向做切口,对比两组患者术后1周、2周、4周时的视力差异、角膜散光度。结果两组患者术前、术后各时期的视力差异均无统计学差异(P>0.05);两组患者术前角膜散光度无统计学差异(P>0.05),术后1周、2周、4周时观察组角膜散光度均显著低于对照组,差异具有统计学意义(P<0.05)。结论选择角膜最大散光子午线轴向做切口行超声乳化白内障吸除联合人工晶状体植入术治疗白内障,能够有效的减少术后的角膜散光度,同时对术后患者视力影响不大,值得在临床上推广和应用。 Objective To investigate the effects of corneal incision at different positions on corneal astigmatism and visual acuity after cataract surgery.Methods A total of 106 cataract patients(106 eyes)admitted to the Third People's Hospital of Yunnan Province from August 2016 to December 2017 were enrolled in this study.All patients were treated with phacoemulsification and intraocular lens implantation.The patients were randomly divided into observation group and control group using digital table method,53 cases in each group.Sacral upper(right eye)and nasal upper(left eye)were selected for corneal incision in control group while the maximal radial meridian axis was chose to make an incision in observation group according to the curvature examination result.The differences in visual acuity and corneal astigmatism were compared between the two groups at 1 week,2 weeks,and 4 weeks post-surgery.Results No significant differences were observed in visual acuity between two groups before and after surgery(P>0.05).No statistical bias was displayed in preoperative corneal astigmatism between the two groups(P>0.05).The corneal astigmatism of the observation group was significantly lower than that of the control group at 1 week,2 weeks,and 4 weeks post-surgery(P<0.05).Conclusion Combination phacoemulsification at the corneal maximal astigmatism meridian axial with intraocular lens implantation for cataract can effectively reduce postoperative corneal astigmatism,and has little impact on postoperative patients'visual acuity,which is worthy of clinical promotion and application.
