摘要
目的探讨腹腔镜胃癌根治术对胃癌患者机体能量代谢及胃肠激素的影响。方法选择2016年7月至2017年6月收治的胃癌根治术患者84例为研究对象,根据本次研究设计原则,采用抽取信封的方式随机分为观察组和对照组各42例。对照组采用开腹胃癌根治术,观察组采用腹腔镜胃癌根治术,比较两组手术相关指标、机体能量代谢、胃肠激素、术后并发症。结果观察组患者手术时间明显长于对照组,术中出血量明显少于对照组,肠鸣音恢复时间、肛门排气时间、住院时间明显短于对照组(均P <0.05);术后3 d时,观察组患者血清白蛋白(ALB)、前白蛋白(PRE)、转铁蛋白(TRF)、视黄醇结合蛋白(RbP)含量明显高于对照组(P <0.05),血清胃泌素(GAS)、胃动素(MTL)、生长抑素(SS)含量明显高于对照组,血管活性肽(VIP)明显低于对照组(P <0.05);切口感染等并发症7.14%明显低于对照组26.19%(χ2=5.486,P <0.05)。结论腹腔镜胃癌根治术有助于减轻手术创伤,促进胃肠功能恢复,减少术后并发症,可能与抑制机体能量代谢、胃肠激素波动等因素有关。
Objective To investigate the effects of laparoscopic radical gastrectomy on energy metabolism and gastrointestinal hormones in patients with gastric cancer.Methods 84 patients with gastric cancer undergoing radical gastrectomy from July 2016 to June 2017 were randomly divided into observation group and control group,according to the design principle of this study.The control group was treated with open radical gastrectomy,while the observation group was treated with laparoscopic radical gastrectomy.Results The operative time of the obser-vation group was significantly longer than that of the control group,and the amount of bleeding during operation was significantly less than that of the control group.The recovery time of bowel sounds,anal exhaust time and hospitalization time of the observation group were significantly shorter than those of the control group(t=10.706,18.737,4.074,5.227,7.773,P<0.05,P<0.01).The contents of albumin(PRE),transferrin(TRF)and retinol binding protein(RbP)in serum were significantly higher than those in control group(t=4.021,2.061,6.796,5.398,P<0.05),and the contents of gastrin(GAS),motilin(MTL)and somatostatin(SS)in serum were signif-icantly higher than those in control group(t=8.109,7.691,5.627,5.359,P<0.05).Complications such as inci-sion infection were 7.14%lower than 26.19%in control group(χ2=5.486,P<0.05).Conclusion Laparoscopic radical gastrectomy is helpful to relieve surgical trauma,promote gastrointestinal function recovery and reduce postoperative complications,which may be related to inhibit energy metabolism and gastrointestinal hormone fluctu-ation.
