摘要
目的探讨碘流率(Iodine Delivery Rate,IDR)及造影用量对肥胖患者颈动脉CTA成像的图像质量影响,以指导造影剂的优化使用。方法将80例临床怀疑颈动脉狭窄或颈动脉斑块的患者随机分为A、B、C、D四组行颈动脉CTA检查,每组20例。A组对比剂(碘比乐300 mgI/mL)用量为50 mL,流速5 mL/s,IDR为1.5 gI/s;B组对比剂(碘比乐300 mgI/mL)用量为50 mL,流速4.5 mL/s,IDR为1.35 gI/s;C组对比剂(碘比乐370 mgI/mL)用量为40 mL,流速4.5 mL/s,IDR为1.67 gI/s;D组对比剂(碘比乐370 mgI/mL)用量为30 mL,流速4.5 mL/s,IDR为1.67 gI/s。分析并比较四组各段颈动脉相同层面CT值,对比噪声比(CNR)、噪声比(SNR),并对各组容积(VR)、曲面重建(CPR)的图像质量进行评分。结果 A、B、C三组间各颈动脉相同层面测量点的CT值没有统计学差异,前三组与第四组颈动脉测量点CT值有统计学差异。四组的CNR、SNR分别为19.25±6.37、23.10±7.15;21.19±5.90、25.38±6.40;21.61±7.60、25.87±7.50;13.39±3.50、17.78±3.96,前三组间没有统计学差异,与第四组存在统计学意义。四组图像重建质量主观评分前三组没有统计学差异,与第四组存在统计学差异,但第四组图像质量仍可满足诊断需要。结论对于肥胖被试,提高注射速度提高IDR可保证图像质量,但也会增加注射风险。本研究采用高浓度造影剂,可以在碘总量不变的情况下提高IDR,不影响图像质量的情况下降低注射速度,降低造影剂风险。
Objective To investigate the effect of iodine delivery rate(IDR)and contrast agent dose on the image quality of carotid artery CT angiography(CTA)imaging in obese patients,so as to guide the optimal use of contrast agents.Methods Eighty patients with clinically suspected carotid stenosis or carotid plaques were randomly divided into 4 groups(20 patients each group)who underwent carotid artery CTA examination.Group A:50 mL dosage of lopamiro 300 mgI/mL,5 mL/s injection velocity,IDR 1.5 gI/s.Group B:50 mL dosage of lopamiro 300 mgI/mL,4.5 mL/s injection velocity,IDR 1.35 gI/s.Group C:40 mL dosage of lopamiro 370 mgI/mL,4.5 mL/s injection velocity,IDR 1.67 gI/s.Group D:30 mL dosage of lopamiro 370 mgI/mL,4.5 mL/s injection velocity,IDR 1.67 gI/s.The CT values of the same carotid artery in four groups were analyzed and compared,the contrast-to-noise ratio(CNR)and noise-signal ratio(SNR)were compared,and the image quality was evaluated.Results There was no significant differences in CT value of carotid artery at the same place of group A,B and C.The CT values of the carotid artery in the first three groups were significantly different with group D.The four groups’CNR and SNR were(19.25±6.37)and(23.10±7.15);(21.19±5.90)and(25.38±6.40);(21.61±7.60)and(25.87±7.50);(13.39±3.50)and(17.78±3.96),respectively.There was no significant difference of CNR and SNR among the first three groups,but they were statistically significant with group D.There were no significant differences of quality of the image reconstruction among the first groups,while they were statistically different with group D,but the image quality of group D could still meet the diagnostic needs.Conclusion For obese subjects,increasing the injection speed can raise IDR and guarantee the quality of the image,but it also will increase the risk of injection.In this study,a high concentration of contrast is used to improve IDR when the total iodine is unchanged.It will not only affect the image quality,but also decrease the injection speed and reduce the risk of contrast media.
