宫腔水造影在子宫及输卵管四维超声造影中的价值 被引量:13

摘要 目的探讨宫腔水造影(Sonohysterograph,SIS)在子宫及输卵管四维超声造影(TVS 4D-HyCoSy)中的临床应用价值。方法选取我院2017年3月至2018年5月子宫输卵管超声造影检查涵盖的200位不孕症患者,对比分析SIS对宫腔病变的显示情况、输卵管伞端通畅性的判断情况及水造影后推注造影剂子宫输卵管分别开始显影时间的差异。结果 SIS联合经阴道三维成像技术(TVS 3D)诊断内膜息肉的敏感度及准确率分别为93.97%、93.18%,明显高于常规方法。SIS后推注造影剂对输卵管伞端粘连诊断的误诊率明显降低,并可以明显缩短输卵管开始显影的时间,差异有统计学意义(P<0.05)。结论 SIS可以在诊断宫腔病变的同时,降低输卵管伞端通畅性判断的误诊率,水造影后再推注造影剂可以明显缩短输卵管开始显影的时间,减少造影剂的使用剂量,提高图像质量及诊断的准确性,操作简单、安全、经济,可作为临床不孕症患者输卵管通畅性检查的一种有效方法。 Objective To investigate the clinical value of saline infusion sonohysterograph(SIS)in TVS and 4D hysterosalpingo-foam sonography(4D-HyCoSy)of uterus and fallopian tubes.Methods From March 2017 to May 2018,a total of 200 infertility patients who underwent hysterosalpingocontrast sonography in our hospital were included in this study.The display of the lesions of the uterine cavity,the patency of the parachute in the end of the fallopian tube and the difference of the development time of the uterine tube in the uterine tube of the SIS were compared.Results The sensitivity and accuracy of SIS combined with TVS 3D in diagnosing endometrial polyps were 93.97%and 93.18%,respectively,which were significantly higher than those of conventional methods.SIS back injection contrast agent significantly reduced the misdiagnosis rate of the diagnosis of oviduct parachute adhesion,and could obviously shorten the time of the oviduct developing,the difference was statistically significant(P<0.05).Conclusion SIS can reduce the misdiagnosis rate of the patency of the fallopian parachute at the same time,reduce the misdiagnosis rate of the patency of the fallopian parachute.The contrast agent can obviously shorten the time of the oviduct to start developing,reduce the dosage of the contrast agent,improve the image quality and the accuracy of diagnosis.It can be used as a simple,safe and economical operation,which can be used as clinical practice.
