Comparison of plasma and serum samples of different biochemical examination items in different detection systems

摘要 目的探讨不同检测系统肝素锂血浆标本和血清标本检测结果的差异及相关性。方法分别在湿化学分析仪雅培ci16000和干化学强生Vitros 5600生化分析仪上测定50例体检健康者的肝素锂抗凝血浆和血清标本的急诊生化项目,对比血浆标本和血清标本急诊生化项目检测结果。结果干化学强生Vitros5600的26项中有16项差异有统计学意义(P<0.05),13个项目的平均偏倚在可接受范围内,而淀粉酶(AMY)、无机磷(P)和二氧化碳(CO2)平均偏倚>1/2CLIA′88要求,但相关性较高(r>0.891 1)。湿化学雅培ci16000的28项中有24项差异有统计学意义(P<0.05),20个项目的平均偏倚在可接受范围内,而P、CO2、钾(K+)和肌酸激酶同工酶(CK-MB)平均偏倚>1/2CLIA′88要求,但相关性高(r>0.927 2)。结论大多数项目在两种生化分析仪上的血浆和血清检测结果的偏倚在可接受范围内。AMY、P、CO2、K+和CK-MB偏倚较大,不具有可比性,需要建立血浆的参考值范围。 Objective To explore the difference and correlation between the results of heparin lithium plasma samples and serum samples from different detection systems.Methods Emergency biochemical items of heparin lithium anticoagulant plasma and serum samples from 50 healthy subjects were measured on wet chemistry Abbott ci16000 and dry chemistry Vitros 5600 detection systems respectively.The results of emergency biochemical items of plasma and serum samples were compared.Results There were statistical differences between the 16 items among 26 items in dry chemical Johnson Vitros 5600(P<0.05).The average bias of the 13 items was within the acceptable range,while the average bias of AMY,P and CO 2 was greater than that of 1/2CLIA′88,but the correlation was good(r>0.891 1).There were statistical differences between the 24 items among 28 items in wet chemical Abbott ci16000(P<0.05),the average bias of the 20 items was within the acceptable range,while the average bias of P,CO 2,K+and CK-MB was greater than that of 1/2CLIA′88,but the correlation was good(r>0.927 2).Conclusion The bias of most of the items in the plasma and serum test results in the two biochemical analyzer are within acceptable limits.The bias of AMY,P,CO 2,K+and CK-MB is not comparable,so it is necessary to establish the reference range of plasma.
