摘要
目的观察呼吸道合胞病毒(RSV)感染喘息组、非RSV感染喘息组及对照组患儿的过敏原、尿白三烯E4(LTE4)及鼻咽分泌物嗜酸性粒细胞阳离子(ECP)的变化水平及其与RSV感染的相关性,探讨过敏原、尿LTE4、ECP在婴幼儿喘息发病机制中的作用。方法免疫印迹法对过敏原进行检测,采用ELISA双抗体夹心法检测尿LTE4、RSV-IgM,Unicap100体外变应原自动检测仪测定NPS-ECP。结果吸入组过敏原阳性率比较,RSV感染喘息组显著高于对照组(P <0.01),而RSV感染喘息组与非RSV感染喘息组比较差异无统计学意义(P> 0.05);非RSV感染喘息组与对照组比较差异无统计学意义(P> 0.05)。RSV感染-喘息组、非RSV感染-喘息组中尿LTE4水平均显著高于对照组(均P <0.01),而RSV感染-喘息组与非RSV感染-喘息组中尿LTE4比较差异无统计学意义(P> 0.05);RSV-感染喘息组、非RSV感染-喘息组NPS-ECP水平均显著高于对照组(均P <0.01),RSV-感染喘息组与非RSV感染-喘息组的NPS-ECP比较差异无统计学意义(P> 0.05)。RSV感染喘息组中尿LTE4水平与NPS-ECP水平呈显著正相关(r=0.752,P <0.01);非RSV感染喘息组中尿LTE4水平与NPS-ECP水平呈显著正相关(r=0.543,P <0.01)。结论吸入组过敏原阳性、尿LTE4、鼻咽分泌物ECP是幼儿喘息发作的重要指标,对婴幼儿喘息的发作、预后有重要意义。
Objective To explore the pathogenesis of infants with wheezing diseases by detecting allergen in-vitro,urine leukotriene E4(LTE4)concentration and eosinophil cationic protein(ECP)in nasopharyngeal secretions,to explore its correlation with respiratory syncytial viral(RSV)infection,and to discuss the role of allergen,LTE4 and ECP in infantile asthmatoid disease.Methods The allergen was detected in wheezing group and control group by immunoblotting.Concentration of urine LTE4 and RSV-IgM were measured by ELISA.The level of nasopharyngeal secretion ECP was measured by Pharmacia UniCAP systems.Results The positive ratio of allergen in wheezing group with RSV-infection was significantly higher than that of the control group(P<0.01),while no statistical significance was observed in wheezing groups with or without RSV-infection(P>0.05)and in wheezing group without RSV-infection and control group(P>0.05).The urine LTE4 concentration of wheezing groups with or without RSV-infection was significantly higher than that of control group(All P<0.01)and the levels of nasopharyngeal secretion ECP of wheezing groups with or without RSV-infection were significantly higher than those of control group(All P<0.01),but no statistical significance was found regarding urine LTE4 concentration and levels of nasopharyngeal secretion ECP in wheezing groups with or without RSV-infection(P>0.05).Urine LTE4 concentration and nasopharyngeal secretion ECP concentration in wheezing group with or without RSV-infection were significantly positively correlated(r=0.752,P<0.01;r=0.543,P<0.01).Conclusion Positive allergen,urine LTE4 and nasopharyngeal secretion ECP levels are important indicators of wheezing attack in children,which provide reference for the treatment and prognosis of infant wheezing.
