摘要
目的评价Gutta Flow常温流动牙胶用于隐裂牙根管治疗一次法联合全冠修复患牙的急性反应及近期疗效,以期为隐裂牙临床根管治疗提供参考。方法随机选择2015年1月～2017年9月我院接收诊治的172例恒磨牙牙隐裂伴牙髓炎患者共200颗牙,按照随机数字表法分为实验组(85例,100颗牙)与对照组(87例,100颗牙)。实验组患牙采用Gutta Flow牙胶+0.06锥度主尖根充行全冠修复;对照组患牙则采取碧蓝糊剂+热牙胶垂直加压充填后行全冠修复。观察并评价两组患者患牙术后7天的急性反应,患牙根尖2、4、6、8mm的位置的充填密合度和随访半年的临床疗效。结果术后7d,实验组患牙急性反应发生率(8.00%)明显低于对照组(25.00%);在距离患者患牙根尖2、4、6、8mm的位置,实验组的充填密合度均显著高于对照组,差异有统计学意义(P <0.05)。随访半年,实验组临床总有效率94.32%显著高于对照组81.79%,差异有统计学意义(P <0.05);结论 Gutta Flow常温流动牙胶一次性根管治疗隐裂牙并联合全冠修复,可以更加严密封闭隐裂磨牙根管系统,减少急性反应发生,提高临床疗效,在隐裂牙临床根管治疗上应用与推广价值均较高。
Objective To evaluate the acute response and short-term effect of single-visit root canal therapy with GuttaFlow combined with complete coronal restoration in cracked teeth,to provide reference for clinical root canal therapy of cracked teeth.Methods A total of200teeth of172cases with permanent molars cracked together with pulpitis in our hospital from January2015to September2017were randomly selected,and they were divided into the experimental group(85cases,100teeth)and the control group(87cases,100teeth)by random number table method.The experimental group was treated with GuttaFlow gutta percha+complete coronal restoration with0.06apical taper root filling,the control group was treated with deep blue cataplasm+complete coronal restoration after hot gutta percha vertical pressure filling.The acute reaction in7days after operation,the filling tightness of2,4,6,8mm apical position and the clinical effect of follow-up for half a year in the two groups were observed and evaluated.Results7days after operation,the incidence of acute reaction in the experimental group(8%)was significantly lower than that(25%)in the control group.At the distances of2,4,6,and8mm from the root tip,the filling tightness of the experimental group was significantly higher than the control group,the difference were statistically significant(P<0.05).After half a year of follow-up,the total effective rate of the experimental group(94.32%)was significantly higher than that(81.79%)of the control group,the difference was statistically significant(P<0.05).Conclusion Single-visit root canal therapy with Gutta Flow combined with complete coronal restoration in cracked teeth can close the root canal system of the cracked molar,reduce the occurrence of acute reaction and improve the clinical effect.The application and popularization value of root canal therapy for cracked teeth are both high.
