摘要
目的:探讨不同手术方式结合植骨治疗桡骨远端C型骨折的临床效果。方法:收治接受钢板内固定联合植骨治疗的桡骨远端C型骨折患者40例为观察组,收治行支架外固定联合植骨治疗的桡骨远端C型骨折患者40例为参考组。比较两组治疗效果。结果:两组术后伤口红肿、破溃及其他炎性反应并发症发生率比较差异无统计学意义(P>0.05),观察组视觉模拟评分(VAS)明显高于参考组(P<0.05),术后功能锻炼时间明显短于参考组(P<0.05);术后1.5年,两组Gartland-Werley功能评分、掌倾角、桡骨高度及尺偏角比较差异无统计学意义(P>0.05);两组随访期畸形愈合、握力下降、旋转受限等后遗症发生率比较差异无统计学意义(P>0.05)。结论:两种术式联合植骨治疗桡骨远端C型骨折均可达到理想效果,钢板内固定联合植骨治疗桡骨远端C型骨折有助于早期功能锻炼,后遗症发生率低;然而其术后可持续长期疼痛,因此在临床治疗时需综合患者病情选择合适治疗方案。
Objective:To explore the clinical effect of different operative methods combined with bone graft in the treatment of distal radius C fractures.Methods:40 patients with distal radius C fractures treated with plate internal fixation combined with bone graft were selected as the observation group,40 patients with distal radius C fractures treated with bracket external fixation combined with bone graft were selected as the reference group,we compared the therapeutic effects between the two groups.Results:There was no significant difference in the incidence of redness,swelling,ulceration and other inflammatory complications between the two groups(P>0.05),the visual analogue scale(VAS)in the observation group was significantly higher than that in the reference group(P<0.05),postoperative exercise time was significantly shorter than that in the reference group(P<0.05).There was no significant difference in Gartland-Werley function score,palmar inclination,radius height and ulnar deviation between the two groups at 1.5 years after operation(P>0.05),during the follow-up period,there was no significant difference in the incidence of malunion,decreased grip strength and limited rotation between the two groups(P>0.05).Conclusion:Two kinds of operation combined with bone grafting can achieve ideal results in the treatment of distal radius C fractures,plate internal fixation combined with bone graft in the treatment of distal radius C fractures was helpful for early functional exercise,the incidence of sequelae was low.However,it can sustain long-term pain after operation,so it was necessary to choose the appropriate treatment plan according to the patient's condition in clinical treatment.
