摘要
目的探讨声脉冲辐射力成像(ARFI)在桥本氏甲状腺炎背景下甲状腺结节良恶性鉴别中的价值。方法回顾性分析211例患者超声检查并经穿刺或手术病理证实的247个桥本氏甲状腺炎背景下甲状腺结节ARFI声像图表现。采用声触诊组织成像(VTI)模式进行二维弹性成像,并分为1~5级;然后采用声触诊组织定量(VTQ)模式定量检测结节内剪切波速度(SWV)。比较良恶性结节VTI和VTQ差别,并形成受试者工作特征曲线(ROC),测量曲线下面积(AUC),决定最佳截断点,计算其敏感性和特异性。结果 247个桥本氏甲状腺炎背景下甲状腺结节中99个为良性,148个为恶性。良恶性结节间VTI(χ2=105. 89,P <0. 01)和VTQ(t=-10. 18,P <0. 01)组间比较差异均具有统计学意义。VTI和VTQ的AUC分别为0. 893和0. 861。以VTI弹性分级3级为截断点,敏感性为87. 8%,特异性84. 8%;以VTQ检测SWV=3. 055 cm/s为截断点,敏感性为79. 7%,特异性为79. 8%。结论 ARFI弹性成像VTI和VTQ技术载桥本氏甲状腺炎背景下甲状腺结节的良恶性鉴别中具有较高价值。
Objective To assess the value of acoustic radiation force imaging(ARFI)in the differentiation of thyroid nodules with the background of Hashimoto thyroiditis.Methods The ultrasonography of247thyroid nodules with Hashimoto thyroiditis in211patients who were pathologically confirmed was retrospectively analyzed.Virtual touch tissue imaging(VTI)mode which graded from1to5were used to evaluate the elasticity of node two-dimensionally,and virtual touch tissue quantification(VTQ)mode was used to detect the shear wave velocity(SWV)quantitatively.The difference of VTI and VTQ between benign and malignant nodules was compared.The receiver operating characteristic curves(ROC)of VTI and VTQ were drawn and the area under curve(AUC)was determined,and the sensitivities and specificities of both were calculated at the optimal cutoff point.Results Among the247thyroid nodules with Hashimoto thyroiditis,99cases were benign,and the other148cases were malignant.There were significant differences in VTI(χ2=105.89,P<0.01)and VTQ(t=-10.18,P<0.01)between the benign nodules and the malignant nodules.The AUCs of VTI and VTQ were0.893and0.861,respectively.The sensitivity and specificity of VTI were87.8%and84.8%,respectively,taking grade3as the cut-off point,while they were79.7%and79.8%in VTQ,respectively,taking SWV=3.055cm/s as the cut-off point.Conclusion ARFI technologies including VTQ and VTI have a good value in the differentiation of thyroid nodules with the background of Hashimoto thyroiditis.
