摘要
目的观察经纤维支气管镜下冲洗治疗慢性阻塞性肺疾病急性加重的老年患者的临床疗效及其对肺功能和血气的影响。方法选取60例2015年1月1日至2017年12月31日在我科住院收治的慢性阻塞性肺疾病急性加重(AECOPD)老年患者,随机分为观察组和对照组,各30例。对照组仅采用常规治疗,而观察组在常规治疗的基础上加用纤维支气管镜下冲洗治疗。治疗72 h后,比较两组患者的临床疗效及治疗前后肺功能和血气的变化。结果观察组的总有效率明显高于对照组,两组比较差异有统计学意义(P<0.05)。治疗后,观察组肺功能指标(FVC、PEF和FEV1)和血气指标(Sa O2和PaCO_2)明显优于同期对照组,两组比较差异有统计学意义(P<0.05)。结论纤维支气管镜下冲洗治疗AECOPD老年患者疗效显著,能够明显改善患者的肺功能和血气状态。
Objective To observe the clinical efficacy of bronchial washings treatment under fiberoptic bronchoscopy on elderly patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and its effects on the variation of pulmonary function and arterial blood gas analysis.Methods 60 elderly patients with AECOPD,who were hospitalized in our department from January 1,2015 to December 31,2017,were randomly divided into observation group and control group(30 cases for each).The control group only used conventional treatment,while the observation group was treated with bronchial washings under fiberoptic bronchoscopy on the basis of conventional treatment.After 72 hours of treatment,the clinical efficacy and changes in pulmonary function and blood gas analysis before and after treatment were compared between the two groups.Results The total effective rate of the observation group were significantly higher than that of the control group.The difference between the two groups was statistically significant(P<0.05).After treatment,the pulmonary function indicators(FVC,PEF and FEV1)and blood gas indicators(SaO2 and PaCO2)were significantly better than those in the control group,and the differences between the two groups were statistically significant(P<0.05).Conclusions bronchial washings treatment under fiberoptic bronchoscopy is effective in treating elderly patients with AECOPD,which can significantly improve pulmonary function and blood gas status of the patients.
