COPD急性加重期患者机械通气脱机困难的影响因素分析 被引量:2

摘要 目的探讨COPD急性加重期患者机械通气脱机困难的影响因素。方法选取研究对象为184例COPD急性加重期患者,研究时间为2016年1月~2017年12月,分为两组,且采用多因素Logistic回归分析机械通气脱机困难的影响因素。结果两组患者在性别、吸烟史、插管前PaO_2、PaCO_2、SaO_2、pH值、白细胞计数、血红蛋白、C反应蛋白、白蛋白对比中无显著差异(P> 0.05);而两组在年龄、插管后白细胞计数、血红蛋白、C反应蛋白、白蛋白、通气时间对比中存在显著差异(P <0.05)。年龄、插管后白细胞计数、插管后血红蛋白、插管后C反应蛋白、插管后白蛋白、通气时间均是机械通气脱机困难的独立危险因素。结论临床医生需严格掌握影响脱机困难因素,通过相应干预对策,以达到降低脱机拔管失败率的目的。 Objective To investigate the influencing factors of mechanical ventilation in patients with acute exacerbation of COPD.Methods A total of 184 patients with acute exacerbation of COPD admitted to our Hospital from January 2016 to December 2017 were randomly divided into control group and observation group.Multivariate logistic regression was used to analyze the influencing factors of mechanical ventilation.Results There were no significant differences in gender,smoking history,PaO2,PaCO2,SaO2,pH,white blood cell count,hemoglobin,C-reactive protein and albumin before intubation(P>0.05).There was a significant difference in white blood cell count,hemoglobin,C-reactive protein,albumin,and ventilation time after intubation(P<0.05).Age,white blood cell count after intubation,hemoglobin after intubation,C-reactive protein after intubation,albumin after intubation,and ventilation time were independent risk factors for mechanical ventilation.Conclusions Clinicians need to strictly control the factors that affect offline difficulty,and through the corresponding intervention measures,to achieve the goal of reducing the rate of offline extubation failure.
