糖尿病患者甲状腺激素与血脂血糖的关系及临床意义 被引量:2

The relationship and clinical significance between thyroid hormone and blood lipid and blood glucose in diabetic patients

摘要 目的研究糖尿病患者甲状腺激素和血脂以及血糖之间的关系,分析其临床意义。方法选择我院收治的102例糖尿病患者,按照患者体内促甲状腺素(TSH)和糖化血红蛋白(HbA_1c)水平的差异来分组,测定患者TSH、血清游离三碘甲状腺原氨酸(FT_3)、血清游离甲状腺素(FT_4)、血脂、空腹血糖(FPG)、HbA_1c等指标情况并进行分析。结果不同性别之间比较,女性患者TSH值高于男性,FT_3、FT_4低于男性(P<0.05);按照HbA_1c水平差异进行分组,随着HbA_1c水平的升高,甲、乙、丙组之间的TSH和FT_3逐渐下降,组间比较差异有统计学意义(P<0.05);按照TSH水平分为A、B、C组,随着TSH水平的提升,3组患者TC、TG以及低密度脂蛋白胆固醇(LDL-C)明显升高,与A组相比,B组和C组升高差异有统计学意义(P<0.05),并且TSH与总胆固醇(TC)、甘油三酯(TG)以及LDL-C呈正相关(P<0.05)。结论测定糖尿病患者甲状腺激素水平,对分析患者病情、判断预后和防治糖尿病并发症有临床价值。 Objective To study the relationship between thyroid hormone,blood lipid and blood glucose in diabetic patients and analyze its clinical significance.Methods One hundred and two diabetic patients admitted to our hospital were selected and divided into three groups according to the differences in thyrotropin,thyroid stimulating hormone(TSH)and hemoglobin A1C(HbA1c)levels in the patients.TSH,free trnodothyronine(FT3),free trnodothyronine(FT4),blood lipid,fasting plasma glucose(FPG),HbA1c and other indicators of the patients were determined and analyzed.Results The TSH value of female patients was higher than that of male patients,and FT3 and FT4 values were lower than those of male patients(P<0.05).According to the differences in HbA1c level,TSH and FT3 between ammong a,b and c group gradually decreased with the increase in HbA1c level,and the differences between groups were significant(P<0.05).According to the TSH level,patients were divided into three groups:group A,group B,and group C.With the increase of TSH level,total cholesterol(TC),triglyceride(TG)and low density lipoprotein cholesterol(LDL-C)were significantly generated in the three groups.Compared with group A,TC,TG,and LDL-C were significantly increased in group B and group C(P<0.05),and TSH,TC,TG and LDL-C were positively correlated(P<0.05).Conclusion The determination of thyroid hormone level in diabetic patients is of high clinical value in the analysis of patients condition,prognosis and prevention and treatment of diabetic complications.
