摘要
目的:探讨逍遥活血汤改善复发性流产患者妊娠结局的作用机理。方法:将60例肝郁脾虚、痰瘀内阻型复发性先兆流产患者随机分为中药组和西药组,每组30例,分别予逍遥活血汤、地屈孕酮片治疗,疗程4周,观察两组临床疗效,彩超观察胚胎发育情况及检测治疗前后血清P、IL-2、IL-4指标的变化。结果:中药组临床疗效明显优于西药组,总有效率为83. 3%,较西药组有显著性差异(P <0. 05)。中药组和西药组治疗后血清孕酮水平上升幅度较治疗前有显著性差异(P <0. 05),两组间无显著性差异(P> 0. 05)。两组治疗后血清IL-2水平均有显著下降,IL-4水平显著上升,中药组与西药组比较有显著性差异(P <0. 01)。结论:逍遥活血汤能改善妊娠结局,可能与其多角度改善内分泌、免疫水平,从而改善肾气-天癸-冲任-胞宫功能有关。
Objective: To investigate the effects of Xiaoyao Huoxue Tang on clinical efficacy and immunoregulation in RSA patients with stagnation of liver-qi and spleen deficiency,phlegm and blood stasis syndrome,exploring its mechanism. Methods: 60 pregnant RSA patients with stagnation of liver-qi and spleen deficiency,phlegm and blood stasis syndrome were randomly divided into Chinese medicine group and Western medicine group,each contained 30 cases,they were given Xiaoyao Huoxue Tang or Dydrogesterone respectively for 4 weeks,then curative effect was appraised,development of fetus was observed by color Duppler ultrasound,blood progesterone level,the serum lever of IL-2 and IL-4 were observed before and after treatment. Results: The clinical efficacy in Chinese medicine group were obviously better than those of the Western medicine group( P <0. 05). Blood progesterone level in both groups did not differ significantly( P > 0. 05). The serum lever of IL-2 in Chinese medicine group was significantly lower than that of the Western medicine group( P < 0. 05),the serum level of IL-4 in Chinese medicine group was significantly. higher than that of the Western medicine group( P <0. 05). Conclusion: Xiaoyao Huoxue Tang can improve pregnancy outcome of RSA patients,regulating endocrine and immunologic function from multi-path to recover the balance of Th1/Th2 was part of its mechanism.
