摘要
目的探究彩色多普勒血流显像(CDFI)技术用于预测脐带绕颈胎儿宫内缺氧及围产结局的临床价值。方法回顾性分析110例孕晚期脐带绕颈胎儿临床资料,根据宫内缺氧情况分为缺氧组(n=43)与非缺氧组(n=67)。比较两组脐动脉(UA)、大脑中动脉(MCA)、肾动脉(RA)血流动力学参数[搏动指数(PI)、阻力指数(RI)、收缩末期/舒张末期峰值血流速度比(S/D)]差异,分析各动脉S/D比值与脐带绕颈匝数、胎心监护正常率、羊水污染分度、剖宫产率、出生后5 min Apgar评分相关性。结果缺氧组UA、MCA及RA在CDFI下各血流动力学参数均明显高于非缺氧组(P <0.05)。Spearman相关性分析结果显示,各动脉S/D比值与脐带绕颈匝数、羊水污染分度均呈显著正相关性(P <0.05),与胎心监护正常率、出生后5min Apgar评分均呈显著负相关性(P <0.05),与剖宫产率无显著相关性(P> 0.05)。结论胎儿各动脉血流动力学参数与胎儿宫内缺氧状况和围产结局密切相关,CDFI对于评估胎儿宫内缺氧及围产结局有较高的临床价值。
Objective To explore the clinical value of color Doppler flow imaging(CDFI)in predicting intrauterine hypoxia and perinatal outcomes of fetuses with umbilical cord around neck.Methods The clinical data of 110 fetuses with umbilical cord around neck in late pregnancy were retrospectively analyzed.According to THE intrauterine hypoxia,the fetuses were divided into a hypoxia group(n=43)and a non-hypoxia group(n=67).The differences of hemodynamic parameters of umbilical artery(UA),middle cerebral artery(MCA)and renal artery(RA),including pulsation index(PI),resistance index(RI)and peak systolic/diastolic flow velocity ratio(S/D)between the two groups were compared,and the correlation between S/D ratio of each artery and the number of umbilical cord around neck,normal rate of fetal heart monitoring,degree of amniotic fluid contamination,cesarean section rate and Apgar score five minutes after the birth was analyzed.Results The hemodynamic parameters of UA,MCA and RA in the hypoxia group were significantly higher than those in the non-hypoxia group(P<0.05).The spearman correlation analysis showed that the S/D ratio of each artery was positively correlated with the number of umbilical cord around neck and the degree of amniotic fluid contamination(P<0.05),negatively correlated with the normal rate of fetal heart monitoring and the Apgar score five minutes after the birth(P<0.05),but not with the cesarean section rate(P>0.05).Conclusion Fetal arterial hemodynamic parameters are closely related to fetal intrauterine hypoxia and perinatal outcomes;CDFI has high clinical value in evaluating fetal intrauterine hypoxia and perinatal outcomes.
