摘要
目的完善退药制度,降低退药率。方法统计医院2016年5月至2017年5月西药房退药处方,对退药原因和退药所属科室等进行分析。结果 1年内门诊处方量为712 992份,退药处方1 022份,占总处方量的0. 14%;退药主要因素为药物录入错误、不良反应、重复开药、服药依从性差等,次要因素为药品缺损和药品近有效期,一般因素为其他购药渠道、方案更改、转入院、药物比例不足等;退药处方最多的科室是消化内科(占13. 01%);退药数量排前10的药品是苯磺酸氨氯地平片、盐酸左氧氟沙星注射液、硫酸氢氯吡格雷片、甲钴胺分散片、甲氨蝶呤片、地氯雷他定分散片、阿莫西林胶囊、铝碳酸镁片、甲硝唑片、氯化钠注射液(100 mL)。结论应健全管理制度,特别是加强对主要退药科室和药物品种的监控,以降低门诊西药房退药率。
Objective To improve the drug withdrawal system and reduce the rate of drug withdrawal. Methods The prescriptions for drug withdrawal in Western pharmacy from May 2016 to May 2017 were collected,the causes of drug withdrawal and the departments those had drug withdrawal were analyzed. Results The number of outpatient prescriptions was 712 992 in 1 year,and 1 022 prescriptions for drug withdrawal,accounting for 0. 14% of the total prescriptions. The main factors for drug withdrawal were incorrect drug entry,adverse reactions, repeated prescribing, and poor adherence to medications and so on. The secondary factors were the proximity of the drug defect and the drug’s effective period. The general factors were other procurement channels,program changes,transfer to hospitals,and insufficient drug proportions and so on. The department with the largest number of drug refund was Department of Gastroenterology( accounting for 13. 01%). The top ten medicines for the number of withdrawals were Amlodipine Besylate Tablets,Levofloxacin Hydrochloride Injection, Clopidogrel Hydrogen Sulfate Tablets, Mecobalamin Dispersible Tablets, Methotrexate Tablets, Desloratadine Dispersible Tablets, Amoxicillin Capsules, Magnesium Aluminate Tablets, Metronidazole Tablets and Sodium Chloride Injection( 100 m L). Conclusion The management system should be improved,especially the monitoring of the main drug withdrawal departments and drug types,in order to reduce the rate of drug withdrawal in outpatient Western pharmacy.
