摘要
目的探讨困难性胆管插管中乳头括约肌预切开术和双导丝插管方案应用对行经内镜逆行性胰胆管造影术(endoscopic retrogradecholangiopancreatography,ERCP)患者成功率及并发症风险的影响。方法研究对象选取我院2015年10月—2018年3月收治行ERCP治疗中胆管插管困难患者共100例,以随机数字表法分为A组(50例)和B组(50例),分别采用乳头括约肌预切开术和双导丝插管技术辅助治疗;比较两组患者治疗成功率和术后并发症发生率。结果 A组和B组患者治疗成功率分别为86.00%(43/50)、64.00%(32/50);A组患者治疗成功率高于B组(P <0.05);A组和B组患者术后并发症发生率分别为8.00%(4/50)、6.00%(3/50);两组患者术后并发症发生率比较差异无统计学意义(P> 0.05)。结论困难性胆管插管中乳头括约肌预切开术和双导丝插管技术方案应用于行ERCP患者总体安全性接近,但治疗成功率乳头括约肌预切开术更具优势。
Objective To investigate the influence of precut sphincterotomy and double filament intubation for difficult bile duct intubation on success rate and complications risk of patients with ERCP.Methods 100 patients with ERCP were chosen from October 2015 to March 2018 in our hospital and divided into A group(50 patients)with precut sphincterotomy and B group(50 patients)with double filament intubation for difficult bile duct intubation;and the treatment success rate and the incidence of postoperative complications of two groups were compared.Results The treatment success rate of A group and B group were separately 86.00%(43/50)and 64.00%(32/50).The treatment success rate of A group were significant higher than that in B group(P<0.05).The incidence of postoperative complications of A group and B group were separately 8.00%(4/50)and 6.00%(3/50).There was no significant difference in the incidence of postoperative complications between two groups(P >0.05).Conclusion Precut sphincterotomy and double filament intubation for difficult bile duct intubation in the treatment of patients with ERCP possess has similar safety;but the application of precut sphincterotomy has higher success rate.
