MRI与CT多期动态增强扫描在肝硬化合并原发性肝癌中的诊断价值比较 被引量:63

Comparison of MRI and CT multi-phase dynamic contrast-enhanced scanning in diagnosis of liver cirrhosis complicated with primary liver cancer

摘要 目的:比较MRI与CT多期动态增强扫描在肝硬化合并原发性肝癌中的诊断价值,为临床诊断与治疗提供有效依据。方法:将院内98例通过病理组织学确诊的肝硬化合并原发性肝癌患者作为研究对象,均行MRI与CT多期动态增强扫描,观察患者病灶数目、大小及强化特点等方面,并计算诊断准确率。采用SPSS 21.0统计学软件进行数据处理。结果:98例患者共113个病灶,其中<1 cm、1 cm≤病灶大小<3 cm、3 cm≤病灶大小<5cm、≥5 cm病灶分别为15、50、38、10个,位于肝左叶37个、肝右叶76个。MRI动脉期以高信号为主,门脉期和延迟期以低信号为主;CT动脉期以高密度特点为主,门脉期和延迟期以低密度或等密度特点为主。MRI扫描病灶可见包膜73个,其中边缘清晰56个,呈现完整环形强化;边缘欠清17个,呈现不完整环形强化。CT扫描病灶可见包膜68个,其中边缘光整49个,呈现明显环形强化;边缘模糊19个,呈现不完整环形强化。CT确诊99个病灶,准确率为87.61%;MRI确诊102个病灶,准确率为90.27%,二者并无明显差异(P>0.05)。结论:相比CT检查,MRI能够多方位、多角度地显示病灶情况,能够更好地显示肝癌血供和包膜强化的特点,因此具有较高的诊断价值,可作为肝硬化合并原发性肝癌的重要诊断手段。 Objective To compare the diagnostic values of magnetic resonance imaging(MRI)and CT multi-phase dynamic contrast-enhanced scanning in the diagnosis of liver cirrhosis complicated with primary liver cancer,so as to provide effective evidences for clinical diagnosis and treatment.Methods Totally 98 cases of liver cirrhosis complicated with primary liver cancer were selected as the research subjects,who were confirmed by the histopathology.The subjects underwent MRI and CT multi-phase dynamic contrast-enhanced scanning and the values of MRI and CT in the diagnosis of liver cirrhosis complicated with primary liver cancer were compared.The clinical data of the 98 patients were input into SPSS 21.0 statistical software for data processing.Results There were 113 lesions found in the 98 cases,of which there were 15 ones smaller than 1 cm,50 ones not smaller than 1 cm while smaller than 3 cm,38 ones not smaller than 3 cm while smaller than 5 cm and 10 ones bigger than 5 cm;there were 37 lesions at the left liver lobe and 76 ones at the right.MRI found 73 cases of capsules,of which there were 56 ones with clear boundaries had obviously circular enhancement and 17 ones with unclear boundaries had incomplete circular enhancement,while CT showed 68 cases of capsules including 49 ones with clear boundaries and obviously circular enhancement and 19 ones with unclear boundaries and incomplete enhancement.CT detected 99 lesions with the accuracy of 87.61%,MRI found 102 lesions with the accuracy of 90.27%,and there no significant difference between the accuracies of CT and MRI(P >0.05).Conclusion Compared with CT,MRI can display lesions in multi-angle and direction,and show liver cancer blood supply and capsule enhancement,so it has a higher diagnostic value and can be used as an important diagnostic method for the diagnosis of liver cirrhosis complicated with primary liver cancer.[Chinese Medical Equipment Journal,2019,40(3):61-63,84]
