摘要
延续性护理是院区护理行为至家庭或社区的延伸,为患者提供相关健康照护问题的解决方案,从而促进患者自我护理能力的提升。本文针对延续性护理的概念、形式、内容、效果等进行综述,作为进一步研究及临床应用的参考。
Continuous nursing is the extension of nursing behavior in the hospital area to the family or community,providing patients with solutions to related health care problems,thus promoting the improvement of patients' self-care ability.This article reviews the concept, form,content and effect of continuous nursing as a reference for further research and clinical application.
