摘要
目的探讨乌灵胶囊联合经颅磁刺激仪治疗脑梗死恢复期认知功能障碍的疗效。方法选取2016年4月～2017年12月在该院神经科门诊治疗的脑梗死恢复期认知功能障碍患者80例,随机分为两组。两组根据病情酌情予以控制血压、血糖和血脂以及抗血小板聚集等基础治疗。观察组予以乌灵胶囊(3粒/次,3次/d,口服)联合经颅磁刺激仪治疗,对照组予以单纯的乌灵胶囊治疗,用法、用量及疗程与观察组完全相同。两组均连用8周。观察两组治疗前后认知功能及日常生活能力的变化,并评估其临床疗效及不良反应。结果治疗8周后,两组WMS、MOCA和BI评分较前显著上升(P<0.05或P<0.01),且观察组治疗后上升值较对照组更明显(P<0.05);同时观察组认知功能改善率较对照组更明显(χ2=5.16,P<0.05)。对照组和观察组分别出现不良反应2例(5.00%)和4例(10.00%),症状较轻,两组不良反应发生率比较差异无统计学意义(χ2=0.18,P>0.05)。结论乌灵胶囊联合经颅磁刺激仪治疗脑梗死恢复期认知功能障碍的疗效较肯定,安全性较好,不仅能更有效地改善患者的认知功能,而且能提高患者的日常生活能力。
Objective To investigate the efficacy of Wuling capsules combined with transcranial magnetic stimulator in the treatment of cognitive dysfunction during cerebral infarction recovery. Methods 80 patients with cognitive dysfunction during cerebral infarction recovery in the neurological outpatient department from April 2016 to December 2017 were selected and randomly divided into two groups. The two groups were given basic treatments such as blood pressure control, blood sugar and blood lipids, and anti-platelet aggregation according to the disease condition. The observation group was given Wuling capsules(3 capsules/time, 3 times/d, orally administered) combined with transcranial magnetic stimulator. The control group was given Wuling capsules alone. The usage, dosage and course of treatment were identical to those in the observation group. Both groups were administered for 8 weeks in a row. The changes of cognitive function and daily living ability before and after treatment were observed in both groups, and the clinical efficacy and adverse reactions were evaluated. Results After 8 weeks of treatment, the WMS, MOCA and BI scores of the two groups were significantly higher than before (P<0.05 or P<0.01), and the increase value in the observation group after treatment was more significant than the control group (P<0.05);at the same time, the cognitive improvement rate in the observation group was more significant than that in the control group(χ^2=5.16, P<0.05). In the control group and the observation group, 2 cases (5.00%) and 4 cases (10.00%) had adverse reactions,and the symptoms were mild. There was no significant difference in the incidence rate of adverse reactions between the two groups(χ^2=0.18, P>0.05). Conclusion Wuling Capsules combined with transcranial magnetic stimulator is effective in the treatment of cognitive dysfunction in the recovery period of cerebral infarction, and the safety is favorable.Not only can it improve the cognitive function of patients more effectively, but also can improve the daily living ability of patients.
