摘要
目的:观察中药药膏外敷联合括约肌间瘘管结扎术(LIFT)治疗复发肛瘘的临床疗效。方法:研究收集2016年2月—2018年2月本院收治复发肛瘘患者120例,按照随机数字表法分为观察组与对照组各60例,对照组采取LIFT手术治疗,观察组采用生肌玉红膏外敷联合LIFT治疗,比较两组术后创面情况、创面愈合率、手术疗效及术后并发症。结果:观察组手术总有效率98.33%,显著高于对照组的88.33%(P<0.05);两组术后第7天、术后第14天疼痛视觉模拟(VAS)评分、创面渗液评分及创面面积均显著低于术后第1天(P<0.05),且观察组不同时间点较对照组改善幅度更大(P<0.05);观察组术后第1周、第2周、第3周创面愈合率均显著高于对照组同时间点(P<0.05);观察组术后并发症发生率5.00%,显著低于对照组的16.67%(P<0.05)。结论:生肌玉红膏联合LIFT治疗复发肛瘘患者,其手术有效率高,可以减轻创面疼痛、渗液,促进创面愈合,降低术后并发症。
Objective:To observe the clinical efficacy of external application of traditional Chinese medicine ointment combined with ligation of intersphincteric fistula tract(LIFT)in the treatment of recurrent anal fistula. Methods:120 cases of patients with recurrent anal fistula admitted to our hospital from February 2016 to February 2018 were collected. According to the random number table method,the patients were divided into observation group and control group,with 60 cases in each group. Control group was treated with LIFT,and observation group was given external application of Shengji Yuhong Plaster combined with LIFT. The postoperative wound conditions,wound healing rate,surgical efficacy and postoperative complications were compared between the two groups. Results:The total effective rate of surgery in observation group was significantly higher than that in control group(98.33% vs 88.33%()P<0.05).The pain visual analogue scale(VAS)score,wound exudation score and wound area at 7 d and 14 d after operation in the two groups were significantly lower than those at 1 d after operation(P<0.05),and the improvement in observation group was greater than that in control group at different time points(P<0.05).The wound healing rates at 1 w,2 w and 3 w after operation in observation group were significantly higher than those in control group(P<0.05). The incidence rate of postoperative complications in observation group was significantly lower than that in control group(5.00% vs 16.67%)(P<0.05). Conclusion:Shengji Yuhong Plaster combined with LIFT for patients with recurrent anal fistula has high surgical effective rate,and it can reduce wound pain and exudation,promote wound healing and reduce postoperative complications.
