摘要
目的探讨血清胱抑素C(CysC)、同型半胱氨酸(Hcy)和尿微量清蛋白/肌酐比值(ACR)联合检测对原发性高血压早期肾损害的诊断价值。方法选取2017年8月至2018年6月该院心内科收治的原发性高血压伴早期肾损害住院患者140例,根据尿液常规检查结果分为尿蛋白阴性组(N组,76例)和尿蛋白阳性组(P组,64例)。选取同期该院门诊健康体检者50例作为健康对照组(NC组)。检测各组研究对象血清CysC、Hcy水平及尿ACR,并比较3项指标联合检测、单独及两两联合检测诊断尿蛋白阴性高血压患者早期肾损害的阳性率。结果 N组、P组患者CysC、Hcy水平及尿ACR均明显高于NC组,且P组患者上述指标明显高于N组,差异均有统计学意义(P<0.05)。N组、P组患者血清Hcy阳性率均高于血清CysC及尿ACR,血清CysC阳性率高于尿ACR。3项指标联合检测诊断尿蛋白阴性高血压患者早期肾损害阳性率高达100%,明显高于单项及两两联合检测。结论血清Hcy对高血压早期肾损害的诊断价值高于血清CysC和尿ACR,3项指标联合检测诊断高血压早期肾损害的价值显著高于单一检测或两两联合检测。
bjective To investigate the value of serum cystatinC(CysC),homocysteine(Hcy) and urinary albumin/creatinine ratio(ACR) combined detection in diagnosing early renal injury of essential hypertension. Methods One hundred and forty inpatients with essential hypertension in the cardiology department of this hospital from July 2017 to June 2018 were selected and divided into the urinary protein negative group(group N,76 cases) and urinary protein positive group(group P, 64 cases) according to the urine routine detection results.Contemporaneous 50 individuals undergoing physical examination in the outpatients department of this hospital were selected as the healthy control group(group NC).The levels of serum CysC,Hcy and urinary ACR were tested in each group.The positive rates for diagnosing early renal injury were compared among the 3-index combined detection,single index detection and pairwise indexes combined detection. Results The levels of serum CysC,Hcy and urinary ACR in the group N and P were significantly higher than those in the group NC, moreover the above indicators in the group P were significantly higher than those in the group N,and the differences were statistically significant( P < 0.05 ).The positive rate of serum Hcy in the group N and P was higher than that of serum CysC and urinary ACR,and the positive rate of serum CysC was higher than urinary ACR.The positive rate of 3-index combined detection for diagnosing early renal injury in the patients with urinary protein negative hypertension reached up to 100%,which was significantly higher than that of single index detection and pairwise indexes combined detection. Conclusion The value of serum Hcy for diagnosing early renal injury of hypertension is higher than that of serum CysC and urinary ACR,and the value of 3-index combined detection for diagnosing early renal injury of hypertension is significantly higher than that of single index detection or pairwise indexes combined detection.
