人工股骨头置换与PFNA内固定治疗高龄不稳定型股骨粗隆间骨折的效果分析被引量：5

摘要目的比较人工股骨头置换术与股骨近端防旋髓内钉(PFNA)内固定治疗高龄不稳定型股骨粗隆间骨折的效果。方法选取2016-04—2018-02间在扶沟县人民医院接受手术的58例高龄不稳定型股骨粗隆间骨折患者。根据不同术式分为2组,各29例。PFNA组实施PFNA内固定术,置换组实施人工股骨头置换术。结果 PFNA组手术时间短于置换组,术中出血量少于置换组;但术后完全负重时间长于置换组、并发症发生率高于置换组。差异均有统计学意义(P<0.05)。2组术后12个月Harris评分差异无统计学意义(P>0.05)。结论 PFNA内固定与人工关节置换治疗高龄不稳定型股骨粗隆间骨折均有确切效果,PFNA内固定手术时间短、创伤小,但术后卧床时间长,并发症多。人工股骨头置换术创伤大,但术后卧床时间短,并发症少。应根据患者具体病情,合理选择手术方式。
