摘要
目的探讨血清同型半胱氨酸(Hcy)和血浆C反应蛋白(CRP)、D-二聚体(D-D)在癫痫患者血中的表达及与服用抗癫痫药物种类、脑电图异常的关系。方法对37例难治性癫痫患者(IE组)、52例抗癫痫药物控制良好者(NE组)和52例健康体检者(对照组)进行血清Hcy和血浆CRP、D-D测定,结合不同脑电图结果及用药种类,探讨血清Hcy和血浆CRP、D-D在癫痫临床诊断及治疗中的意义。结果 IE组患者血清Hcy和血浆CRP、D-D水平均明显高于对照组和NE组(P <0.05),NE组患者血清Hcy和血浆CRP、D-D水平均明显高于对照组(P <0.05);脑电图重度异常组患者血清Hcy和血浆CRP水平均明显高于脑电图正常组及轻度、中度异常组(P <0.05),并且随着脑电图异常逐渐严重,血清Hcy、血浆CRP水平逐渐增高(P <0.05),血浆D-D在不同脑电图组中的表达差异无统计学意义(P> 0.05);服用3种抗癫痫药物的血清Hcy水平高于服用2种及1种抗癫痫药物(P <0.05),服用2种抗癫痫药物的血清Hcy水平高于服用1种抗癫痫药物(P <0.05),3种、2种、1种用药组血浆CRP及D-D结果差异无统计学意义(P> 0.05)。结论血清Hcy和血浆CRP、D-D在癫痫患者血清中呈高表达;高血清Hcy,血浆CRP水平与脑电图异常程度有关;服用多种抗癫痫药物可升高血清Hcy水平。
Objective Investigation of the levels of serum homocysteine(Hcy),plasma C reactive pro tein( CRP) and D-Dimer(D-D) in patients with epilepsy,as well as their relationship with the types of anti epilep tic drugs used and abnormal EEG. Methods 37 cases of intractable epilepsy( IE group), 52 of good antiepileptic drug response( NE group) and 52 of health examination( control group) were employed. The levels of serum Hcy, plasma CRP and D-D were determined and their significance in the diagnosis and treatment of intractable epilepsy was studied. Results The value of serum Hcy,plasma CRP,and D-D in IE group were significantly higher than that of NE group and control group(P<0.05). The value of serum Hcy,plasma CRP and D-D in NE group were significantly higher than that of control group(P<0.05). However,the value of serum Hcy and plasma CRP in patients with severe abnormal EEGwere significantly higher than those with normal,m ild and moderate abnormal EEG groups(P<0.05). Moreover,The value of serum Hcy and plasma CRP levels gradually elevated with increas ing abnormality in EEG(P<0.05). No significant difference was noted in plasma DD between different types of EEG groups(P > 0.05). The value of serum Hcy in patients who took three types of antiepileptic drugs is higher than those with two or one types of drug(P<0.05), and the value of serum Hcy in patients who took two types of antiepileptic drugs was higher than those took one types. There was no significant difference in plasma CRP and D-D levels among patients with three,two and one types of antiepileptic drugs(P > 0.05). Conclusions The expression level of serum Hcy,plasma CRP,and D-D is high in epileptic patients,the elevated serum Hcy,plasma CRP levels are highly related to EEG abnormalities,and taking different types of antiepileptic drugs can increase expression level of serum Hcy.
