摘要
目的探讨分析SCN5A基因多态性与糖尿病心脏病变患者左室功能的关系。方法选取102例糖尿病合并心脏病变患者(T2DM+HD组)、96例单纯T2DM患者(T2DM组)及60例健康人员(NC组),分析3组SCN5A基因C5457T位点基因多态性,随访分析不同基因分型患者心血管事件发生情况。结果T2DM+HD组高血压史、HbA1c、TG、LDL-C、HDL-C、hs-CRP、FFA、VE/VA、IVST、LVMI、LVEF、LVEDD,与T2DM组比较差异有统计学意义(P <0.05或P <0.01)。T2DM+HD组TT基因型、T等位基因频率高于NC组、T2DM组,差异有统计学意义(χ~2=20.678、23.135、9.677、10.747,P <0.01)。Logistic回归分析,高血压史、HbA1c、TG、HDL-C、hs-CRP、FFA、IVST、LVMI为T2DM+HD的影响因素。TT型患者TG、LDL-C、HbA1c、hs-CRP、FFA、IVST、LVMI,与CC型、CT型比较差异有统计学意义(P <0.05或P <0.01)。TT型患者出现心血管事件累积发生风险与CC型、CT型患者比较,差异有统计学意义(Log χ~2=5.586、4.763,P=0.015、0.027)。结论 SCN5A基因C5457T位点多态性与糖尿病心脏病变患者左室功能障碍有一定相关性,TT基因型患者心血管事件发生率高。
Objective To investigate the relationship between SCN5A gene polymorphism and left ventric ular function in patients with diabetic heart disease. Methods 102 patients with diabetes mellitus complicated with heart disease( T2DM + HD group), 96 patients with T2DM alone( T2DM group) and 60 healthy people( NC group) were selected to analyze the polymorphism of C5457T locus in three groups of SCN5A gene. Analysis of car diovascular events in patients with different genotyping. Results Hypertensive history,H bA1c,TG,LDL-C, HDL- C,hs-CRP,FFA,V E/VA,IVST,LVMI,LVEF,LVEDD in T2DM + HD group were statistically signifi cant compared with T2DM group(P<0.05 or P<0.01). The frequency of TT genotype and T allele in T2DM + HD group was higher than that in NC group and T2DM group,the difference was statistically significant(χ^2= 20.678,23.135,9.677,10.747,P<0.01). Logistic regression analysis showed that hypertension history, HbA1c,TG,H DL C,hs CRP,FFA,IVST and LVMI were influencing factors for T2DM + HD. There were signif icant differences in TG,LDL C,H bA1c,hs CRP,FFA,IVST and LVMI between TT and CC,CT(P<0.05 or P<0.01). The risk of cardiovascular events in patients with TT was significantly different from those of CC and CT patients( Log χ^2 = 5.586,4.763,P = 0.015,0.027). Conclusion The C5457T polymorphism of SCN5A gene is associated with left ventricular dysfunction in patients with diabetic heart disease. The incidence of cardiovascular events in patients with TT genotype is high.
