摘要
目的分析上消化道出血患者院内病死率及死亡原因。方法回顾性分析2012年1月至2018年1月首都医科大学附属北京友谊医院收治的1 103例因上消化道出血住院的患者的临床资料,包括性别、年龄、住院天数、主要诊断、是否行内镜检查及治疗、是否入住监护室、是否输血及院内死亡等。结果 1 103例上消化道出血患者中,共74例患者死亡,院内病死率为6. 71%(74/1 103),死亡原因分别为胃及胆道系统恶性肿瘤、肝脏功能衰竭、心脏功能衰竭、呼吸衰竭及失血性休克等。按死亡原因分组比较,心、肺功能衰竭组患者年龄最大,肝脏功能衰竭组患者年龄最小。结论恶性肿瘤、脏器功能衰竭等非出血原因已成为上消化道出血的主要死因;因肝脏功能衰竭合并上消化道出血死亡的患者年龄最小,应成为临床关注的重点。
Objective To analyze the in-hospital mortality of patients with upper gastrointestinal bleeding and causes of death.Methods A total of 1103 patients with upper gastrointestinal bleeding in this hospital during January 2012 to January 2018 were enrolled..Clinical informations including gender,age,duration of hospitalization,main diagnosis,endoscopy and endoscopic therapy,duration for ICU staying,amount of blood transfusion,mortality and all these data were collected and retrospectively analyzed.Results Among these 1 103 patients,74 cases died in-hospital(with mortality rate of 6.71%).The causes of death including gastric cancer or malignant tumor in biliary tract system,liver failure,heart failure,respiratory failure and hemorrhagic shock.According to the causes of death,patients with heart or lung failure were the eldest and those with liver failure were the youngest.Conclusion Non-bleeding-related death such as malignant tumors and organ failure have become the main cause of death for upper gastrointestinal bleeding;the mean age of patients died of liver function failure is the youngest,they should be the focus for clinical attention.
