摘要
目的评价CT冠脉血管成像技术诊断心绞痛患者的临床价值。方法选取2017年2月～2018年2月本院收治的107例疑似心绞痛患者,所有患者均实施CT冠脉血管成像技术诊断及冠状动脉造影诊断,并以冠状动脉造影作为金标准,分析CT冠脉血管成像技术在心绞痛诊断中的准确度、灵敏度、特异度。结果 107例疑似患者经冠状动脉造影确诊为心绞痛者88例,非心绞痛者19例;其中88例心绞痛中包括重度狭窄32例,中度狭窄35例,轻度狭窄21例。107例疑似患者经CT冠脉血管成像技术确诊为心绞痛者85例,非心绞痛者22例;其中85例心绞痛中包括重度狭窄30例,中度狭窄34例,轻度狭窄21例。CT冠脉血管成像技术诊断检出率为79.4%(85/107),诊断符合率为96.59%(85/88)。CT冠脉血管成像技术诊断符合率与金标准比较,差异无统计学意义(P>0.05)。CT冠脉血管成像技术诊断灵敏度为100.00%,特异度为86.36%。结论在心绞痛诊断中,应用CT冠脉血管成像技术可有效鉴别心绞痛病变情况,为临床诊治提供有效的参考依据,且其属于无创检查,检查方便、简单、安全,受到医生及患者的普遍认可,可作为心绞痛最常用的一种诊断手段,值得临床推广应用。
Objective To evaluate the clinical value of CT coronary angiography in the diagnosis of angina pectoris.Methods A total of 107 patients with suspected angina pectoris admitted to our hospital from February 2017 to February 2018 were selected.All patients underwent CT coronary angiography diagnosis and coronary angiography diagnosis,and coronary angiography was used as the gold standard to analyze the diagnostic accuracy,sensitivity and specificity of CT coronary angiography in angina pectoris.Results Of the 107 patients with suspected angina pectoris,88 cases were diagnosed as angina pectoris and 19 cases were non-angina pectoris by coronary angiography.Among them,88 patients with angina pectoris included 32 patients with severe stenosis,35 patients with moderate stenosis,and 21 patients with mild stenosis.Of the 107 suspected patients,85 patients with angina pectoris and 22 patients with non-angina pectoris were diagnosed by CT coronary angiography.Among them,85 patients with angina pectoris included 30 patients with severe stenosis,34 patients with moderate stenosis,and 21 patients with mild stenosis.The diagnostic detection rate of CT coronary angiography was 79.4%(85/107),and the diagnostic coincidence rate was 96.59%(85/88).The diagnostic coincidence rate of CT coronary angiography was not statistically significant compared with the gold standard(P>0.05).The diagnostic sensitivity of CT coronary angiography was 100.00%,the specificity was 86.36%.Conclusion In the diagnosis of angina pectoris,CT coronary angiography can effectively identify angina pectoris,provide an effective reference for clinical diagnosis and treatment,and it is non-invasive,easy,simple and safe to check,widely recognized by doctors and patients.It can be used as a diagnostic tool most commonly used for angina pectoris.It is worthy of clinical promotion and application.
