摘要
目的探讨黄浦区社区综合康复管理模式对脑卒中患者康复效果的影响。方法选取223例黄浦区2016年7月—2017年6月新发病的脑卒中患者,分为干预组(137例)与对照组(86例),2组患者均给予常规治疗,另对干预组患者开展为期6个月的肢体康复训练、健康宣教及心理疏导。所有研究对象定期由康复指导师进行量表评估,并完成2次相关知识问卷调查。结果干预组患者的Barthel指数(BI)、Berg分值(BBS)、知识知晓程度及PHQ-9得分改善程度均优于对照组(P<0.05)。知识知晓水平的提升幅度与性别、年龄和文化程度有关,女性、高龄及低文化程度者提升幅度大(P<0.05),知晓程度的提升程度与患者抑郁症状的改善程度呈现正相关(β=1.6,P=0.010)。结论开展社区脑卒中患者综合康复管理,可有效帮助患者恢复基本生活能力、降低跌倒风险并缓解抑郁症状,提高其生活质量。
Objective To discuss the effect of comprehensive community rehabilitation management on stroke cases in Huangpu District,Shanghai.Methods 233 cases with new-onset stroke during the period from July,2016 to June,2017 were selected and divided into 2 groups:intervention group(n=137)and control group(n=86);routine treatment was performed on all the cases in the 2 groups while limb rehabilitation training,health education and psychological guidance for 6 month were added to cases in intervention group;all the cases received regular scale assessments under the guidance of rehabilitation instructors and filled questionnaires on relevant knowledge twice.Results The improvement in Barthel index(BI),Berg scores,knowledge status and PHQ-9 scores in intervention group was superior to that in control group(P<0.05);the increase of knowledge level was correlated to gender,age and education degree,which was greater in female,the aged and the cases of lower education degree(P<0.05);the increase of relevant knowledge statue was in positive correlation with the improvement of depression symptoms of the cases(β=1.6,P=0.010).Conclusions The performance of comprehensive community rehabilitation management can effectively help the recovery of basic living ability of the stroke cases,decrease the risk of falling,relieve the depression symptoms and raise the quality of life.
