磁共振T_2 map成像评估国际骨循环研究会Ⅱ期坏死股骨头软骨及骨微结构损伤状况的应用价值 被引量:1

Application value of magnetic resonance T_2 map imaging for assessing the injury status of avulsed femoral bone cartilage and bone microstructures in ARCOⅡ stage

摘要 目的通过对国际骨循环研究会(ARCO)Ⅱ期股骨头坏死组和正常对照组关节软骨、骨微结构的磁共振(MR)T_2map进行形态学、定量学分析评估其临床应用价值。方法收集确诊20髋股骨头坏死(ARCOⅡ期)为试验组及20髋正常股骨头为对照组,比较2组软骨、骨微结构的磁共振成像(MRI)、T_2map伪彩图形态学结构变化及定量学指标。结果 2组间软骨及骨微结构的T_2map定量学指标差异无统计学意义(P>0.05),但坏死组关节软骨T_2map值总体稍高。2组间软骨及骨微结构在MRI、T_2map伪彩图的形态结构表现不同。结论正常股骨头关节软骨的T_2map的数值可界定为(50±8),临床上可将此区间值作为可能性参考标准,同时通过形态学主要观察髋关节MR及T_2map伪彩图软骨、骨微结构形态及颜色变化亦有一定价值。 Objective By morphological and quantitative analysis of MR T2 map of articular cartilage and bone microstructure of ARCO stageⅡfemoral head necrosis group and normal control group,its clinical application value was evaluated.Methods Twenty hips with femoral head necrosis(ARCOⅡstage)as experimental group and 20 hips with normal femoral heads as control group were collected,the morphological changes of the MRI and T2 maps of the cartilage and bone micro-structures in the two groups were compared and quantification indicators were compared.Results T2 map quantitative indicators of cartilage and bone microstructure were not statistically different between two groups,P>0.05,but the T2 map value of joint cartilage in necrosis group was slightly higher.The morphological features of cartilage and bone micro structures in MRI and T2 map were different between the two groups.Conclusion The T2 map value of normal femoral articular cartilage can be defined as approximately(50±8).This interval value can be used clinically as a possible reference standard,at the same time,through the morphological observation of hip joint MR and T2 map pseudo-color map cartilage,bone microstructure and color changes also have some value.
