胰岛素样生长因子-Ⅰ和血管内皮生长因子在子宫内膜异位症诊断中的价值 被引量:12

The value of insulin-like growth factor-I and vascular endothelial growth factor on the diagnosis of endometriosis

摘要 目的探讨胰岛素样生长因子-Ⅰ(insulin-like growth factor-I,IGF-Ⅰ)和血管内皮生长因子(vascular endothelial growth factor,VEGF)在子宫内膜异位症诊断中的价值。方法选择2012年1月至2017年6月武汉市普仁医院妇产科诊治的80例子宫内膜异位症患者和同期健康体检的80例女性作为研究对象,将80例子宫内膜异位症患者设为研究组,将同期健康体检的80例女性设为对照组。采用ELISA法测定两组人员血清IGF-Ⅰ和VEGF水平。结果研究组患者血清IGF-Ⅰ和VEGF水平均高于对照组(均P<0.05)。子宫内膜异位症Ⅲ-Ⅳ期血清IGF-Ⅰ和VEGF水平均高于Ⅰ-Ⅱ期(均P<0.05)。子宫内膜异位症患者痛经评分与血清IGF-Ⅰ和VEGF水平均无显著相关性(均P<0.05)。子宫内膜异位症患者血清IGF-Ⅰ和VEGF水平呈正相关(P<0.05)。血清IGF-Ⅰ诊断子宫内膜异位症的灵敏度、特异度分别为78.8%、83.8%,血清VEGF诊断子宫内膜异位症的灵敏度、特异度分别为81.3%、88.8%,血清IGF-Ⅰ和VEGF均阳性诊断子宫内膜异位症的灵敏度、特异度分别为63.8%、98.8%,血清IGF-Ⅰ和VEGF一项阳性诊断子宫内膜异位症的灵敏度、特异度分别为96.3%、60.0%。结论子宫内膜异位症患者血清IGF-Ⅰ和VEGF水平升高,血清IGF-Ⅰ和VEGF水平与病情严重程度有关,在子宫内膜异位症诊断中具有一定价值,两者联合检测可以提高子宫内膜异位症的诊断灵敏度和特异度。 ObjectiveTo investigate the value of insulin-like growth factor-I (IGF-I) and vascular endothelial growth factor (VEGF) on the diagnosis of endometriosis. Methods80 patients with endometriosis diagnosed in the Department of Obstetrics and Gynecology of Wuhan Puren Hospital from January 2012 to June 2017 were selected as the study group, and 80 women received physical examination at the same time were selected as the control group. Serum levels of IGF-I and VEGF of patients in the two groups were measured by ELISA. ResultsThe serum levels of IGF-I and VEGF in the study group were higher than those in the control group (all P<0.05). The level of serum IGF- I and VEGF in patients with stage III-IV of endometriosis were higher than that of patients with stage I -II of endometriosis (all P<0.05). There were no significant correlation between dysmenorrhea score and the levels of serum IGF-I and VEGF in patients with endometriosis (all P<0.05). There was positive correlation between serum IGF-I and VEGF levels in patients with endometriosis (P<0.05). The sensitivity and specificity of serum IGF-I in the diagnosis of endometriosis was 78.8% and 83.8%. The sensitivity and specificity of serum VEGF in diagnosis of endometriosis were 81.3%, 88.8%. The sensitivity and specificity of serum IGF-I joint with VEGF positive in the diagnosis of endometriosis were 63.8%, 98.8%. The sensitivity and specificity of either serum IGF-I or VEGF positive in the diagnosis of endometriosis were 96.3% and 60.0% respectively. ConclusionsThe serum IGF-I and VEGF levels in patients with endometriosis are increased. Serum IGF-I and VEGF levels are associated with severity of the disease, and the combined detection can improve the diagnostic sensitivity and specificity of endometriosis.
