摘要
目的探讨肝细胞癌(HCC)中细胞核增殖抗原(Ki-67)和凋亡相关因子配体(FasL)的表达关系及临床意义。方法收集本院78例HCC组织及癌旁组织,同时取18例肝脏正常组织作为对照。链霉菌抗生物素蛋白-过氧化物酶(SP)免疫组织化学法检测组织标本中Ki-67和FasL表达;结合临床资料分析Ki-67、FasL与HCC患者临床病理特征及预后的关系。应用SPSS 20.0统计软件进行分析。结果SP免疫组织化学结果显示,Ki-67主要表达于HCC组织细胞核内,其在HCC组织中阳性表达率为89.74%,显著高于癌旁组织(47.44%,χ^2=32.402,P<0.05)和正常组织(5.56%,χ^2=53.820,P<0.05)。FasL主要表达于HCC组织细胞质内,其在HCC组织中阳性表达率为67.95%,显著高于癌旁组织(39.74,χ^2=12.484,P<0.05)和正常组织(11.11%,χ^2=19.309,P<0.05)。Ki-67、FasL表达强度与HCC患者的病理分级、淋巴结是否转移有关(P<0.05)。至随访结束,全部患者的中位生存时间为50.00个月,术后1、3、5年累积生存率分别为82.05%、60.26%、47.44%。Ki-67高表达患者术后1、3、5年的累积生存率分别为75.51%、48.98%、38.78%,Ki-67低表达患者分别为93.10%、79.31%、62.07%,两者生存率差异有统计学意义(P<0.05)。FasL高表达患者术后1、3、5年的累积生存率分别为71.88%、43.75%、31.25%,FasL低表达患者分别为89.13%、73.91%、58.70%,两者生存率的差异有统计学意义(P<0.05)。结论Ki-67和FasL表达水平与HCC的发生、发展明显相关。
Objective To explore the expression relationship between proliferation cell nuclear antigen(Ki-67)and factor associated suicide ligand(FasL)in hepatocellular carcinoma(HCC)and its clinical significance.Methods 78 cases of HCC tissues and para-carcinoma tissues were collected from June 2008 to June 2012,and 18 normal liver tissues were taken as controls at the same time.Streptavid-in-peroxidase(SP)immunohistochemical method were used to detect the Ki-67 and FasL expression in tissue samples;analyze the relationship between Ki-67,FasL and clinicopathological features and prognosis of HCC patients combined the clinical data.Statisticalanalysis of data using the statistical product and service solutions 20.0 software.Results(1)SP immunohistochemistry results showed that Ki-67 was mainly expressed in the nuclei of HCC tissues,and the positive expression rate in HCC tissues(89.74%)was significantly higher than that in adjacent tissues(47.44%,χ^2=32.402,P<0.05)and normal tissues(5.56%,χ^2=53.820,P<0.05).(2)FasL was mainly expressed in cytoplasm of HCC tissue,and its positive expression rate in HCC tissues(67.95%)was significantly higher than that in adjacent tissues(39.74%,χ^2=12.484,P<0.05)and normal tissues(11.11%,χ^2=19.309,P<0.05).(3)The expression intensity of Ki-67 and FasL was related to the pathological grade and lymph node metastasis of HCC patients(P<0.05).(4)At the end of follow-up,the median survival time of all the patients was 50.00 months,and the cumulative survival rates were 82.05%,60.26%and 47.44%respectively at 1,3 and 5 years after operation.The cumulative survival rates of patients with Ki-67 high expression were 75.51%,48.98%and 38.78%respectively at 1,3 and 5 years after operation;the cumulative survival rates of patients with Ki-67 low expression were 93.10%,79.31%and 62.07%respectively;and the survival rates of the two groups were significantly different(P<0.05).The cumulative survival rates of patients with FasL high expression were 71.88%,43.75%and 31.25%respectively at 1,3 and 5 years after operation.the cumulative survival rates of patients with FasL low expression were 89.13%,73.91%and 58.70%respectively,and the survival rates of the two groups were significantly different(P<0.05).Conclusion The expression levels of FasL and HCC is related to the occurrence and development of Ki-67,and it can be used to evaluate the clinical prognosis of patients with HCC.
