大血管旁部位小肝癌的精准消融治疗 被引量:10

Ultrasound-guided precision percutaneous microwave ablation for small hepatocellular carcinoma adjacent to large vessels

摘要 目的评估超声引导下微波消融(MWA)大血管旁小肝癌病灶的疗效,探讨该类病灶精准消融治疗方法。方法收集武汉同济医院2010年1月到2017年7月86例经皮MWA治疗位于大血管旁小肝癌病灶的临床数据,采用回顾性分析,统计并发症发生情况、局部疗效、生存预后情况。结果 86个病灶的完全消融率94.2%(81/86)。随访8~89个月,随访期累计18例局部复发,23例死亡。治疗后1、3和5年的局部无复发率是:82.3%、76.7%和68.1%。原发肝癌治疗后1、3和5年的无瘤生存率是:61.6%、35.7%和22.0%;1、3和5年的累计生存率是:94.3%、72.3%和63.3%。治疗过程均未出现明显并发症。结论超声引导下经皮MWA治疗大血管旁小肝癌可获得精准、良好、安全的疗效。 Objective To evaluate the curative efficacy of ultrasound-guided precision percutaneous microwave ablation(PMWA) in treating small hepatocellular carcinoma(HCC) adjacent to large vessels, and to discuss the precision ablation therapy technique for such lesions. Methods The clinical data of 86 patients with small HCC adjacent to large vessels, who were admitted to the Affiliated Tongji Hospital of Tongji Medical College of China to receive PMWA during the period from January 2010 to July 2017, were collected. The complications, local curative efficacy, survival and prognosis were retrospectively calculated and statistically analyzed. Results The complete ablation rate of the 86 lesions was 94.2%(81/86). The patients were followed up for 8-89 months. During the follow-up period, 18 patients developed local recurrence and 23 patients died. The one-, 3-and 5-year local non-recurrence rates were 82.3%, 76.7%and 68.1% respectively. Postoperative one-, 3-and 5-year disease-free survival(DFS) rates were 61.6%,35.7% and 22.0% respectively. The one-, 3-and 5-year cumulative survival rates were 94.3%, 72.3% and63.3% respectively. No obvious complications occurred in the course of treatment. Conclusion In treating small HCC adjacent to large vessels, ultrasound-guided PMWA is clinically safe and it can obtain precise and reliable curative effect.
